Abstract [en]

Background: 56.8 million people around the world are in need of palliative care. Patients´ suffering, stressful work and internal conflicts present nurses with ethical dilemmas in care situations. Increased knowledge about nurses´ experiences of being faced with ethical dilemmas is therefore essential to facilitate person-centered care.

Aim: The aim of this study was to describe nurses´ experiences of being faced with ethical dilemmas in late palliative care of adults in inpatient care.

Method: The study was based on a qualitative literative study, with eight scientific studies, which were selected from the databases Cinahl and PubMed. Delimitations in the form of peer reviewed, english language, all adults and published between 2015-2025 were used. Friberg´s (2022) five- step model was used to analyze the studies.

Results: Two themes emerged from the analyses: Weight of compassion and Work environment- opportunities and obstacles. This study found that nurses in inpatient palliative care experienced moral distress and inner conflict when facing ethical dilemmas, eased by collegial support and a trusting team.

Conclusion: Nurses in inpatient palliative care face ethically challenging situations that may cause emotional strain and moral uncertainty. Supportive teamwork and opportunities for reflection are important to support nurses in navigating ethical dilemmas and maintaining patient dignity at the end of life.

Behovet av palliativ vård ökar världen över och sjuksköterskor kan hamna i situationer där olika viljor, värderingar och förväntningar krockar. Syftet med denna studie var att beskriva sjuksköterskors erfarenheter av att ställas inför etiska dilemman vid sen palliativ vård av vuxna inom slutenvården.

Studien visar att sjuksköterskor kan uppleva moralisk stress och inre konflikter när de förväntas agera på sätt som går emot deras yrkesetik eller patienters vilja. Arbetsmiljön har stor betydelse då ett tryggt och stödjande team kan bidra till att sjuksköterskor känner sig mer säkra i sitt arbete. Bakgrunden presenterar de vårdvetenskapliga begreppen lidande och vårdande, relevanta till studiens syfte.

Studien är litteraturbaserad och bygger på åtta vetenskapliga studier, funna i databaserna Cinahl och PubMed, vilka analyserats med hjälp av Fribergs femstegsmodell. Resultatet presenterade två teman och fyra subteman. Resultatet diskuteras med valda vårdvetenskapliga begrepp och ny forskning, vilket stärker resultatet.
