Bakgrund: I den psykiatriska vården blir komplementära metoder allt vanligare. Det finns dock utmaningar vid införande av komplementära metoder (KAM) som utgör hinder i form av negativa föreställningar och attityder kopplade till normer och organisatorisk struktur. I vårdens utveckling har specialistsjuksköterskan en central roll för att kvalitetssäkra nya evidensbaserade omvårdnadsinsatser.

Syftet: Syftet var att beskriva vårdpersonalens erfarenheter av komplementära metoder i psykiatrisk vård, utifrån specialistsjuksköterskans perspektiv.

Metod: En metasyntes med etnografisk metod genomfördes. Nio kvalitativa vetenskapliga artiklar analyserades enligt Noblit och Hares sju steg. Resultatet syntetiserades tematiskt och diskuterades i ljuset av den salutogena teorin för att få en djupare förståelse för de hälsofrämjande aspekterna av KAM inom psykiatrisk omvårdnad.

Resultat: Det framkom tre teman. ”Holistiskt förhållningssätt i vårdrelationen”, ”Meningsfull omvårdnad främjar egenvård” samt ”Kompetent teamsamverkan för hållbar vård”. Det identifierades sex subteman att se hela den unika människan, att visa engagerad närvaro i ögonblicket, att skapa begriplighet genom att vara i nuet, att få ökad förmåga att hantera utmaningar, att trotsa tvivel vid otillräcklig kunskap samt att skapa förutsättningar för ett hälsofrämjande arbetsklimat. Av resultatens teman och subteman formades syntesen ”En vårdkultur i med- och motvind”.

Slutsats: Resultatet visade på en vårdkultur med osynliga barriärer vid användning av KAM. Vårdkulturen präglades av normer och förhållningssätt, vilket kunde vara både främjande och hämmande. Specialistsjuksköterskan inom psykiatrisk vård bör kontinuerligt arbeta med normer för att möjliggöra hälsofrämjande arbete med KAM. Det kan i sin tur bidra till att KAM ger förutsättningar för en mer holistisk och meningsfull omvårdnad.

Background: Complementary methods (CAM) are becoming increasingly common in psychiatric care. However, their implementation is challenged by prevailing norms and organizational structures. In the development of healthcare, the specialist nurse plays a central role in ensuring the quality of new evidence-based nursing interventions.Aim: The aim was to describe healthcare professionals’ experiences of complementary methods in psychiatric care, from the perspective of the specialist nurse’s responsibility in nursing.

Method: A meta-synthesis with an ethnographic approach was conducted. Nine qualitative scientific articles were analyzed using Noblit and Hare’s seven-step model. The results were thematically synthesized and discussed in the light of the salutogenic theory to gain a deeper understanding of the health-promoting aspects of complementary methods in psychiatric nursing.

Results: Three main themes emerged: “A holistic approach in the care relationship”, “Meaningful nursing promotes self-care”, and “Competent team collaboration for sustainable care”. Six subthemes were identified: seeing the whole unique person, showing engaged presence in the moment, creating comprehensibility by being in the present, gaining an increased ability to manage challenges, overcoming doubt in the face of insufficient knowledge, and creating conditions for a health-promoting work climate. In the main and subthemes, the synthesis formed a care culture in headwind and tailwind.

Conclusion: It revealed a care culture with invisible barriers to the use of CAM. The culture was shaped by norms and approaches, which could both facilitate and hinder. The psychiatric mental health nurse should continuously work with prevailing norms to enable health-promoting work with CAM. This may, in turn, allow CAM to create conditions for more holistic and meaningful nursing care.
