Bakgrund:

Sjuksköterskor inom intensivvård har en central roll i patientens vård och befinner sig nära både patienten och närstående. Beslutsprocessen vid avveckling av intensivvård innebär komplexa medicinska och etiska beslut. Patienterna riskerar att utsättas för meningslös intensivvård, vilket kan leda till en upplevelse av etisk stress för sjuksköterskan.

Syfte:

Att sammanställa sjuksköterskors erfarenheter av beslutsprocessen vid avveckling av intensivvård.

Metod:

En systematisk litteraturstudie genomfördes enligt Bettany-Saltikov och McSherry (2019). Den systematiska sökningen utfördes i PubMed, Cinahl och PsycINFO och resulterade i 22 inkluderade artiklar som kvalitetsgranskades och analyserades genom innehållsanalys.

Resultat:

Analysen visade två kategorier: Hinder i beslutsprocessen och underlättande faktorer i beslutsprocessen. Kategorin hinder i beslutsprocessen innefattade tre underkategorier. De beskrev erfarenheter av när beslutsprocessen uteblev, försenades och när den dominerades av ett ensidigt medicinskt perspektiv. Det framkom erfarenheter av hierarkiska strukturer, meningsskiljaktigheter, kulturella och religiösa skillnader vilket påverkade beslutsprocessen negativt. Sjuksköterskorna beskrev erfarenheter av att delta i meningslös intensivvård på grund av utebliven eller fördröjd beslutsprocess och att det ökade etisk stress. Kategorin underlättande faktorer i beslutsprocessen innefattade två underkategorier. De beskrev sjuksköterskors informella och formella delaktighet i beslutsprocessen och kommunikationens betydelse. Erfarenheter av hur sjuksköterskor formellt inkluderades i beslutsprocessen i form av vårdkonferenser framkom, och hur lång erfarenhet gav informellt inflytande i beslutsprocessen. Vidare beskrevs god kommunikation, samarbete och ömsesidig tillit som centralt för en fungerande beslutsprocess.

Slutsats:

Sjuksköterskors erfarenheter beskriver vikten av en tydlig och inkluderande beslutsprocess vid avveckling av intensivvård där kommunikation och samarbete lyfts fram som viktiga faktorer. Att stärka sjuksköterskors delaktighet i beslutsprocessen kan bidra till minskad etisk stress och en vård som respekterar patientens värdighet och autonomi.

Background:

Intensive care nurses have a central role in patient care and are in close contact with both patients and their families. The decision-making process regarding the withdrawal of intensive care involves complex medical and ethical considerations. There is a risk that patients may be subjected to futile care, which may contribute to nurses experiencing ethical stress.

Aim: 

To explore nurses experiences of the decision-making process regarding the withdrawal of intensive care.

Method:

A systematic literature review was conducted using the guideline of Bettany-Saltikov and McSherry (2019). Systematic searches were performed in PubMed, Cinahl and PsycINFO, resulting in 22 included articles that were quality-assessed and analyzed using content analysis

Results:

The analysis revealed two categories: Barriers in the decision-making process and facilitating factors in the decision-making process. The category barriers in the decision-making process include three subcategories. These described experiences of when the decision-making process was absent, delayed, or dominated by a one-sided medical perspective. Nurses reported experiences of hierarchical structures, disagreements, and cultural and religious differences that negatively influenced the decision-making process. They also described experiences of participating in futile care due to absent or delayed decision-making, which increased moral distress among nurses. The category facilitating factors in the decision-making process included two subcategories. These described nurses’ informal and formal participation in the decision-making process and the importance of communication. Experiences were reported where nurses were formally included through care conferences, and where long clinical experience granted informal influence in the decision-making process. Furthermore, good communication, collaboration, and mutual trust were described as central to a well-functioning decision-making process.

Conclusion:

Nurses’ experiences highlight the importance of an explicit and inclusive decision-making process in the withdrawal of intensive care, with communication and collaboration as key factors. Strengthening nurses’ involvement in the decision-making process may help reduce moral distress and support care that respect the patients dignity and autonomy.
