Bakgrund:

Ungdomsvården i Sverige styrs av barnkonventionen och nationell lagstiftning, där barnets bästa och rätt till delaktighet utgör centrala principer. Syftet med vården är att erbjuda trygghet och behandlingsstruktur, men forskning visar att låsta och institutionella miljöer kan förvärra psykisk ohälsa – särskilt hos redan utsatta ungdomar. Detta understryker vikten av att utgå från ungdomars egna upplevelser och att tillämpa ett vårdvetenskapligt perspektiv med fokus på känsla av sammanhang, salutogena principer och ett helhetsperspektiv.

Syfte:

Studiens syfte är att undersöka ungdomars egna upplevelser av att vårdas inom låsbara enheter i Sverige.

Metod:

Studien är en systematisk litteraturöversikt med induktiv ansats, baserad på artiklar publicerade mellan åren 2015 och 2024. Systematiska databassökningar genomfördes i PsycINFO, Scopus, Cinahl och Criminal Justice. Urvalet bestod av åtta vetenskapliga artiklar med fokus på ungdomars egna röster. Vidare utfördes en tematisk analys för att identifiera mönster utifrån ungdomarnas upplevelser.

Resultat:

Den tematiska analysen resulterade i två huvudteman och sex subteman. Det första temat, ”Kontrollförlusten – bortom vuxnas förståelse”, beskriver hur ungdomar upplever den fysiska miljön som begränsande, personalens befogenheter som integritetskränkande, och hur lydnad ses som en strategi för frihet – trots en oviss framtid. Det andra temat, ”Mellan terapeutiska och skadliga relationer”, belyser relationer till personal, andra ungdomar och familj – där omsorg och våld ofta samexisterar. Temana speglar en vardag präglad av maktobalans, motstånd och sökandet efter trygghet.

Slutsats:

Att vårdas på låsta enheter innebär för många ungdomar en verklighet präglad av kontrollförlust, identitetsförskjutning och en ständig kamp för att bli sedda. Genom att utgå från ungdomars perspektiv och känsla av sammanhang kan vården bli mer meningsfull, trygg och individanpassad, vilket är avgörande för målgruppens långsiktiga återhämtning.

Background:

Youth care in Sweden is governed by the convention on the rights of the child and national legislation, where the best interests of the child and the right to participation are central principles. In cases of serious risk to a child’s health or development, compulsory care under the care of young person’s act may be applied, often within secure units managed by the National Board of Institutional Care. These environments aim to provide safety but, according to research, can also exacerbate mental health problems in vulnerable youth. There is therefore a growing need for care based on young people's subjective experiences, sense of coherence, and a holistic, salutogenic perspective.

Aim:

The aim of this study is to explore young people's experiences of being cared for in locked institutional units in Sweden.

Method:

An inductive, systematic literature review based on eight scientific articles identified through searches in PsycINFO, Scopus, Cinahl, and Criminal Justice. The articles were published within the years 2015 and 2024. A thematic analysis was used to identify patterns in youth experiences.

Results:

Two main themes emerged. (1) “Loss of Control – Beyond Adult Understanding” describes how youth experience a restrictive environment, lack of autonomy, and how obedience becomes a strategy to navigate the system. (2) “Between Therapeutic and Harmful Relationships” highlights how relationships with staff and peers are shaped by both care and violence, where safety and resistance coexist.

Conclusion:

Life in secure units is marked by loss of control, identity struggles, and power imbalance. Grounding care in young people’s own experiences and the sense of coherence makes it more meaningful, safer, and person-centred – which is crucial for long-term healing and recovery.
