Bakgrund:

Svårläkta sår är ett hälsoproblem som drabbar framförallt äldre och multisjuka. I takt med stigande andelen äldre ses svårläkta sår som ett globalt hot. Personcentrerad vård, teamsamverkan, adekvat sårbedömning, kontinuitet och korrekt kunskap om sårläkningsprocessen är faktorer som ligger till grund för en god sårläkning. Distriktssköterskan har en central roll i vårdandet av patienter med svårläkta sår eftersom det är ett tillstånd som innebär lidande för patienter.

Syftet:

Syftet med studien var att beskriva distriktssköterskors upplevelser av vilka utmaningar de möter i vårdandet av patienter med svårläkta sår.

Metoden:

Metoden som användes var en integrativ litteraturstudie med fokus på kvalitativa studier.

Resultat:

Analysen resulterade i fyra teman med sju subteman. De mest framträdande fynden var behov av fortbildning, teamsamverkan och användning av evidensbaserade kliniska riktlinjer. Tidsbrist, kunskapsbrist är de främsta utmaningar distriktssköterskor möter i vårdandet av patienter med svårläkta sår.

Slutsats:

Denna studie belyser distriktssköterskors upplevelser av utmaningar i vården av patienter med svårläkta sår. Huvudfynden visar att distriktssköterskor möter flera sammanlänkade utmaningar på organisations-, team- och individnivå. Dessa utmaningar påverkar deras förmåga att tillämpa evidensbaserad omvårdnad och kräver en helhetssyn för att erbjuda effektiv och säker vård. Trots utmaningarna ser distriktssköterskor vårdandet av svårläkta sår som en viktig del av sitt ansvar och använder motivation och vilja för att erbjuda bästa möjliga vård för patienterna.

Background:

Hard-to-heal wounds are more likely to affect the elderly and people with multiple illnesses. As the percentage of the elderly increases, difficult-to-heal wounds are seen as a global threat. Person-centered care, team collaboration, adequate wound assessment, continuity and knowledge of the wound healing process are factors that underlie good wound healing. The district nurse has a central role in the care of patients with hard-to-heal wounds because it is a condition that involves suffering for patients.

Aim:

The aim of this review was to describe district nurses' experience of the challenges they face in caring for patients with hard-to-heal wounds.

Method:

An integrative literature study was conducted with focus on a qualitative approach guided by the aim of the study.

Results:

The analysis resulted in four themes with seven subthemes. The most prominent findings are the need for continuing education, team collaboration and use of evidence-based clinical guidelines. Lack of time and lack of knowledge are the main challenges district nurses face in caring for patients with difficult-to-heal wounds.

Conclusion:

This literature review has identified and highlighted district nurses' experiences of challenges in caring for patients with chronic wounds. The main findings show that district nurses encounter several interconnected challenges affecting their ability to apply evidence-based care. These challenges can be categorized into organizational factors, team-related issues, the pursuit of knowledge and education, and the promotion of patient involvement. All these challenges are closely linked and influence each other, emphasizing the need for a holistic approach to address them in order to provide effective and safe care. Despite the perceived difficulties, district nurses view wound care as a significant part of their nursing responsibility. Motivation and dedication are strategies employed by district nurses to manage these challenges and deliver the best possible care to their patients.
