The conversation of life in the waiting room of death : A review about nurses preceived difficulties during serious illness conversations (English)
Abstract [sv]
Bakgrund: Palliativ vård syftar till att förbättra livskvalitén och lindra lidande för patienter med livshotande sjukdom. För att patienter med palliativa vårdbehov ska erhålla vård som överensstämmer med deras önskemål och värderingar är det viktigt att samtal vid allvarlig sjukdom hålls. Sjuksköterskor kan hålla dessa samtal, och förmåga till god kommunikation är viktigt då samtalet ska anpassas efter patientens enskilda behov och önskemål. Tidigare forskning påvisar positiva effekter för patienter av att ha samtal om livets slut, såsom ökat hopp och större känsla av kontroll. Samtidigt visar tidigare forskning att sjuksköterskor är av uppfattningen att det inte är deras roll att hålla samtalet och det finns en rädsla att prata om livets slut, varpå samtalet ofta undviks.
Syfte: Att kartlägga och sammanställa svårigheter sjuksköterskor upplever i samtal vid allvarlig sjukdom.
Metod: En systematisk litteraturstudie med kvalitativ ansats vars metodologiska riktlinjer utgår ifrån Bettany-Saltikov och McSherrys metod. Systematiska sökningar utfördes i Cinahl och PubMed som slutligen resulterade i elva relevanta och kvalitetsgranskade artiklar. Genom innehållsanalysen framkom tre teman och åtta subteman som bildade resultatet.
Resultat: Tre huvudteman framkom; Samtalets oförutsägbarhet, Att möta patienten där den är och Samtalets premisser. Sjuksköterskans upplevda svårigheter i samtal vid allvarlig sjukdom grundar sig i olika faktorer. Att känna sig otillräcklig som sjuksköterska och en rädsla att framkalla och hantera känslor framkommer som en försvårande faktor. Det upplevs svårt när patienten inte vill samtala om livets sista tid, eller när patienten saknar sjukdomsinsikt. Samtalet kan även hämmas av patientens fysiska mående. Organisatoriska faktorer såsom tidsbrist och arbetsmiljö kan försvåra samtalet, men också bristande teamarbete och otydligt rollansvar påvisades vara bidragande i sjuksköterskors upplevda svårigheter.
Slutsats: Sjuksköterskans upplevda svårigheter kring samtal vid allvarlig sjukdom är komplexa och påverkas av flera faktorer. Det är viktigt att sjuksköterskan får stöd och utbildning för att hantera dessa utmaningar.
Abstract [en]
Background: Palliative care seeks to improve the quality of life and alleviate suffering for patients with life-threatening illness. In order for patients with palliative care needs to receive care that is consistent with their preferences and values, it is important to have a serious illness conversation. Nurses can hold these conversations, and the ability to communicate well is important as the conversation must be adapted to the patient's individual needs and wishes. Previous research indicates positive outcomes from serious illness conversations, such as increased hope and a greater sense of control. At the same time, previous research shows that nurses believe it is not their role to hold the conversation and a fear of addressing end-of-life topics, leading to their avoidance.
Aim: The aim of this systematic literature study was to map and compile difficulties nurses experience when having serious illness conversations.
Method: A systematic literature review with a qualitative approach, following the methodological guidelines proposed by Bettany-Saltikov and McSherry, was conducted. Systematic searches were performed in Cinahl and PubMed, resulting in the identification of eleven relevant and quality-reviewed articles. Through content analysis, tree main themes and eight subthemes became visible, forming the study’s findings.
Results: Tree main themes appeared; The conversations unpredictability, To meet the patient where he is and The premises of the conversations. The nurse's perceived difficulties when having serious illness conversations are based on various factors. The unpredictability of the conversation, including feelings of inadequacy as a nurse and apprehension about eliciting and managing emotions, emerge as aggravating factors. Furthermore, the results show that the nurses find it difficult when the patient does not want to discuss end-of-life matters or lack insight into their illness. The conversation can also be hindered by the patient's physical condition. Organizational factors, such as lack of time and the work environment, contribute to the complexity of these conversations. Additionally, unclear role responsibilities and a lack of teamwork were shown to contribute to nurses' perceived difficulties.
Conclusion: The nurse’s perceived challenges in having serious illness conversations are multifaceted and influenced by various factors. It is crucial for the nurse to receive adequate support and training to effectively address these challenges
