Abstract [en]

The global prevalence of diabetes is increasing, placing demands on healthcare systems as they work to meet the needs of this growing patient group. Diabetes specialist nurses play a central role in supporting these patients and contributing to positive health outcomes, yet knowledge remains limited regarding the prerequisites that enable them to fulfil their professional responsibilities. This compilation dissertation aimed to explore professionals’ and patients’ experiences of contemporary diabetes healthcare with a specific focus on the prerequisites for person-centred care, work-integrated learning and job satisfaction among diabetes specialist nurses.

A multimethod design combining qualitative and quantitative methods was employed. Data were collected through interviews with patients (study I), surveys (studies II and III) and focus-group discussions with diabetes specialist nurses (study IV). The analyses included qualitative content analysis (study I), statistical analyses (studies II and III) and focus-group analysis (study IV).

The findings reveal an increasingly strained diabetes healthcare system. Patients expressed a desire for multidimensional, person-centred diabetes care (study I), while diabetes specialist nurses reported striving to maintain their professionalism and person-centred practice despite a strained care environment characterised by increasing pressures and high workloads along with a perception that diabetes care is deprioritised (study IV). Study II examined how the learning potential of the workplace was associated with diabetes specialist nurses’ perceptions of providing person-centred care and identified two contributing factors: ‘support in learning’ and the ‘opportunity to reflect’. Study III revealed a direct relationship between diabetes specialist nurses’ occupational self-efficacy and job satisfaction and showed that the workplace’s learning potential—‘time for exploration’—mediated this relationship. This dissertation concludes that, in an increasingly strained diabetes healthcare system, investing in work-integrated learning and supportive organisational structures is essential for ensuring person-centred diabetes care and strengthening diabetes specialist nurses’ job satisfaction.

Udio osoba s dijabetesom globalno raste, što stavlja velike zahtjeve na sposobnost zdravstvene zaštite da zadovolji njihove potrebe. Osobe s dijabetesom imaju povećan rizik od kardiovaskularnih komplikacija i predstavljaju značajan udio korištenja zdravstvenih usluga. Dijabetološke medicinske sestre igraju centralnu ulogu u podršci ovim pacijentima i mogu doprinijeti poboljšanju zdravstvenih rezultata. Međutim, nedostaje im dovoljno znanja o preduslovima za obavljanje svoje profesionalne prakse kako bi ispunile svoj zadatak.

Svrha ove kumulativne disertacije je bila istražiti iskustva stručnjaka i pacijenata u suvremenom liječenju dijabetesa, s posebnim fokusom na uslove za liječenje usmjereno na osobu, učenje integrirano u rad i zadovoljstvo poslom među dijabetološkim medicinskim sestrama.

U ovoj disertaciji je korišten više metodski pristup koji kombinira kvalitativne i kvantitativne metode. Podaci su prikupljeni putem intervjua s pacijentima (Studija I); anketnih ispitivanja (Studije II i III) i fokus grupa (Studija IV) s dijabetološkim medicinskim sestrama. Podaci su analizirani korištenjem kvalitativne analize sadržaja (Studija I), statističkih analiza (Studije II i III) i analiza fokus grupa (Studija IV).

Studija I je ispitala kako pacijenti doživljavaju trenutno liječenje dijabetesa i koje su njihove potrebe za učenjem i podrškom. Pacijenti su izrazili želju za višedimenzionalnim i na osobu usmjerenom liječenjem dijabetesa – liječenjem koja uzima u obzir cijelu njihovu životnu situaciju, potrebe za učenjem i izazove koje dijabetes sa sobom nosi, gdje je dijabetološka medicinska sestra često opisivana kao saveznik. Istovremeno, je više njih opisalo da je liječenje dijabetesa koje danas primaju ograničeno i da ne zadovoljava uvijek njihove lične potrebe.

Studija II je ispitivala kako okolina za učenje na radnom mjestu utiče na mogućnosti dijabetoloških medicinskih sestara da rade na način usmjeren na osobu. Utvrđeno je da su dva faktora posebno važna: podrška u svakodnevnom učenju i mogućnosti za refleksiju. Kada dijabetološke medicinske sestre doživljavaju da mogu učiti na poslu i imaju vremena da razmisle o svom radu, one također ocjenjuju da je njega koju pružaju više usmjerena na osobu. To ukazuje da kada se profesionalcima omogući učenje u radnoj sredini to može doprinijeti njezi usmjerenoj na osobu za osobe s dijabetesom.

Studija III je istraživala odnose između samopouzdanja na poslu, potencijala za učenje na radnom mjestu i zadovoljstva poslom testiranjem hipotetičkog modela strukturnih jednačina. Rezultati su pokazali direktnu vezu između samopouzdanja na poslu dijabetoloških medicinskih sestara i zadovoljstva poslom. Jedna dimenzija okoline za učenje na radnom mjestu – imati vremena za istraživanje u svakodnevnom radu – pozitivno utiče na ovaj odnos.

Studija IV je istraživala iskustva dijabetoloških medicinskih sestara o uslovima za njegu usmjerenu na osobu i njihov profesionalni rad u primarnoj zdravstvenoj zaštiti dijabetičara. Rezultati pokazuju da medicinske sestre nastoje održati svoj profesionalizam, uprkos opterećenoj zdravstvenoj sredini koju karakterizira sve veći pritisak za promjenama i veliko opterećenje poslom gdje se doživljava da liječenje dijabetesa im nizak prioritet.

Zaključno, disertacija pokazuje da je, u sve opterećenijem sistemu zdravstvene zaštite za dijabetes, ključno stvoriti uslove za učenje integrirano u rad i podržavajuće organizacijske strukture kako bi se osiguralo liječenje dijabetesa usmjereno na osobu i poticalo zadovoljstvo poslom dijabetoloških medicinskih sestara.

Andelen personer med diabetes ökar globalt, vilket ställer stora krav på hälso- och sjukvårdens förmåga att möta deras behov. Personer med diabetes har en förhöjd risk för kardiovaskulära komplikationer och står för en betydande del av vårdutnyttjandet. Diabetessjuksköterskor spelar en central roll i att stödja dessa patienter och kan bidra till förbättrade hälsoutfall. Det saknas dock tillräcklig kunskap om vilka förutsättningar de har i sin yrkesutövning för att fullfölja sitt uppdrag.

Syftet med denna sammanläggningsavhandling var att utforska professionellas och patienters erfarenheter av den nutida diabetessjukvården, med särskilt fokus på förutsättningar för personcentrerad vård, arbetsintegrerat lärande och arbetstillfredsställelse bland diabetes specialistsjuksköterskor.

En multimetodansats som kombinerar kvalitativa och kvantitativa metoder användes i denna avhandling. Data samlades in genom intervjuer med patienter (Studie I); enkätundersökningar (Studier II och III) och fokusgruppsdiskussioner (Studie IV) med diabetesspecialistsjuksköterskor. Data analyserades med kvalitativ innehållsanalys (Studie I), statistiska analyser (Studier II och III) och fokusgruppsanalys (Studie IV).

I studie I undersöktes hur patienter upplever dagens diabetesvård och vilka lärande- och stödbehov de har. Patienterna uttryckte ett önskemål om en multidimensionell och personcentrerad diabetesvård – en vård som tar hänsyn till hela deras livssituation, lärandebehov och de utmaningar som diabetes innebär, där diabetessjuksköterskan ofta beskrevs som en allierad. Samtidigt beskrev flera att den diabetesvård de får idag är begränsad och inte alltid möter deras personliga behov.

I studie II undersöktes hur arbetsplatsens lärandemiljö påverkar diabetessjuksköterskors möjligheter att arbeta personcentrerat. Två faktorer visade sig vara särskilt viktiga: stöd i det dagliga lärandet och möjlighet till reflektion. När diabetessjuksköterskor upplever att de kan lära på jobbet och får tid att reflektera kring sitt arbete, skattar de också vården de bedriver som mer personcentrerad. Det tyder på att lärande i arbetsmiljö för de professionella kan bidra till personcentrerad vård för personer med diabetes.

I studie III undersöktes sambanden mellan arbetssjälvtillit, arbetsplatsens lärandepotential och arbetstillfredsställelse genom att testa en hypotetisk strukturell ekvationsmodell. Resultaten visade en direkt relation mellan diabetessjuksköterskors arbetssjälvtillit och arbetstillfredsställelse. En dimension av arbetsplatsens lärandemiljö – att ha tid för utforskande i det dagliga arbetet – inverkar positivt på detta samband.

I studie IV utforskades diabetessjuksköterskors erfarenheter av förutsättningarna för personcentrerad vård samt deras professionella arbete i den primärvårdsbaserade diabetesvården. Resultaten visar att sjuksköterskorna strävar efter att upprätthålla sin professionalitet, trots en ansträngd vårdmiljö som kännetecknas av ökande förändringstryck och hög arbetsbelastning där diabetesvården upplevs nedprioriterad.

Sammanfattningsvis visar avhandlingen att det, i en alltmer ansträngd diabetes-hälso- och sjukvård, är avgörande att skapa förutsättningar för arbetsintegrerat lärande och stödjande organisatoriska strukturer för att säkerställa en personcentrerad diabetesvård och för att främja diabetessjuksköterskors arbetstillfredsställelse.

Abstract [en]

OBJECTIVE: The objective of this study was to explore patients' learning and support needs within contemporary diabetes care to help them deal with daily life challenges.

DESIGN: A qualitative descriptive design was used following the Consolidated Criteria for Reporting Qualitative Studies. The 15 individual face-to-face interviews were analysed using qualitative content analysis.

SETTING: Participants were drawn from three healthcare centres in rural and urban regions of West Sweden.

PARTICIPANTS: The study involved 15 patients (8 men, 7 women) with T2DM who experienced contemporary diabetes care.

RESULTS: Patients expressed a strong desire for access to person-centred, multidimensional learning, with a focus on genuine partnership, tailored education, and emotional engagement. Digital tools were seen as valuable aids in their self-care efforts. Sub-themes were 'Desiring genuine partnership and tailored patient education' and 'Needing support related to altered perspectives on life and awareness of care standards but with finite care resources'.

CONCLUSION: The findings suggest that integrating person-centred, multidimensional learning strategies into diabetes care could be beneficial, particularly when addressing both practical and emotional needs. Encouraging active patient engagement through flexible digital solutions and providing support for emotional well-being may improve the overall patient experience. However, further research and practical application are needed to fully understand how these strategies could be effectively implemented to support patients with T2DM in managing their daily health challenges.

Abstract [en]

Diabetes mellitus presents global health challenges, thereby increasing the need for person-centred care. This cross-sectional study of 157 diabetes specialist nurses evaluated the theoretical structure of the Swedish version of the Learning potential of the workplace scale and explored whether the learning potential of the workplace, work engagement, quantitative demands, influence, stress and work experience in diabetes care were associated with nurses’ perceptions of person-centred care. Regression analysis showed that the variables “opportunity to reflect”, “support in learning’, “work engagement” and “work experience in diabetes care” were positively associated with perceived person-centred care. Perceived organisational and environmental support for person-centred care was positively linked to “opportunity to reflect”, “support in learning” and “influence” and negatively to “stress” and “quantitative demands”. Reporting followed the STROBE guidelines. The findings demonstrated that healthcare systems can enhance nurses’ ability to provide person-centred care for persons with diabetes by fostering learning-oriented workplaces and addressing organisational challenges.

Abstract [en]

Background: Diabetes mellitus is a chronic condition, cases of which are expected to continue rising worldwide. Diabetes specialist nurses play an essential role by assisting patients with preventing or delaying disease complications. Research has suggested an association between occupational self-efficacy and job satisfaction among professionals. However, the relationship between these factors among diabetes specialist nurses, as well as the learning potential of the workplace in this context, remains unexplored.

Objective: This study aimed to explore the relationships between occupational self-efficacy, the learning potential of the workplace and job satisfaction.

Design: A cross-sectional study design was used.

Setting: The study data were collected through a national online survey conducted in Sweden.

Participants: A total of 157 registered nurses who provide diabetes care to patients were included.

Methods: Data were obtained through an online survey with a response rate of 28%. All variables were measured using Swedish-translated standardised instruments. The model was constructed and tested using structural equation modeling analysis with the hypothesis that perceived occupational self-efficacy has both direct and indirect effects on diabetes specialist nurses’ job satisfaction and is mediated by the learning potential of the workplace.

Results: The findings supported the hypothesised model. The total effect of the relationship between occupational self-efficacy and job satisfaction was 0.547 ( p < .001), comprising both the direct effect (β = 0.359, p < .0001) between these constructs and the indirect effect (0.188, p < .001). The indirect pathways included occupational self-efficacy, which was statistically significantly associated with all three dimensions of the Learning Potential of the Workplace scale: Opportunity to reflect (β = 0.480, p < .0001), Support in learning (β = 0.226, p < .01), and Time for exploration (β = 0.330, p < .0001). Both Support in learning and Time for exploration were also statistically significantly associated with job satisfaction (β = 0.236 and β = 0.266, respectively, p < .01), thereby contributing to the sum of the indirect effect (0.188, p < .001). One dimension, Time for exploration , was identified as a mediator between occupational self-efficacy and job satisfaction, which explained 9% ( p < .01) of the variance in job satisfaction.

Conclusions: These results demonstrated the association between occupational self-efficacy and job satisfaction among diabetes specialist nurses. The mediation effect of the Time for exploration dimension underscores the importance of providing sufficient time for workplace learning. This finding suggests that fostering a supportive learning environment could be associated with job satisfaction.

IntroductionThe prevalence of type 2 diabetes is increasing alarmingly worldwide. Evidence consistently shows that nurse-led care by diabetes specialist nurses is crucial for delivering efficient front-line primary diabetes management. This study aimed to explore diabetes specialist nurses' experiences of the prerequisites for person-centred care and for their professional practice in primary diabetes healthcare.MethodsA total of 16 diabetes specialist nurses from western Sweden participated in four focus-group discussions during May 2025. The data were analysed to identify common themes among the participants' experiences according to Krueger and Casey's descriptions.ResultsThe analysis resulted in one main theme: "maintaining professionalism in a changing and strained primary diabetes care". This theme included five subthemes that described diabetes specialist nurses' experiences regarding the prerequisites for person-centred care and professional practice. The diabetes specialist nurses reported daily challenges in delivering person-centred care in contemporary primary diabetes healthcare. Their professional commitment was evident in their clinical practice and in their efforts to maintain professional competence. Despite emphasising the importance of long-term, person-centred patient engagement in diabetes care, they face limited time to interact with patients and insufficient managerial support for their professional learning.DiscussionThe findings underscore diabetes specialist nurses' need for organisational support, particularly from managers who value and actively promote person-centred care, as well as opportunities for continuous professional development. Taken together, such supportive measures could lay the groundwork for a sustainable primary healthcare system for diabetes in the future.
