Abstract [en]

Longitudinal studies of neurocognition in schizophrenia spectrum disorders (SSD) usually follow relatively young first-episode patients across several years. Comparatively little is known about the neurocognitive trajectory of samples also consisting of older patients. This is a 20-year follow-up study of participants who performed the baseline assessment at different ages and utilizes data from the Swedish Clinical Long-Term Psychosis Study (CLIPS). At baseline, 61 SSD patients were included and available for clinical assessment after 20 years. Of these, 28 performed neurocognitive assessment at both baseline and 20 years later. The test results from this group were used for this study. After 20 years, the participants exhibited significantly worsening cognitive flexibility, verbal learning, verbal retention memory, and verbal intellectual function compared to baseline. All the statistically significant differences from baseline to follow-up had large effect sizes. The other cognitive domains showed no statistically significant changes from baseline for either group. We conclude that although the overall picture was one of neurocognitive stability across 20 years, our participants showed signs of accelerated ageing in the verbal domain specifically.

Abstract [en]

Background/Objectives: Workplace violence (WV) against healthcare workers (HCWs) is a major hazard all over the world. Prevention requires a reliable risk assessment. The rate of HCWs reporting a violent event varies considerably across multi-year retrospective studies compared to periodic surveys. We conducted a rapid observational study to demonstrate that data collection methods are more important than socio-cultural and healthcare organizational differences in determining the frequency of reported violence. Methods: In June 2025, in a cross-sectional observational comparison, we examined a total of 236 nurses divided into three groups: the first two were recruited online from Brescia (Italy) and Trollhättan (Sweden), while the third group was composed of Latium (Italy) nurses participating in a sleep health promotion program who answered the same questions on WV online. All the workers reported the frequency of violent incidents experienced in the previous 12 months using the Violent Incident Form (VIF), occupational stress using the Effort/Reward Imbalance questionnaire (ERI), and work ability via the Work Ability Score (WAS). Results: In the three samples, WV was correlated positively with stress and inversely with work ability (p < 0.01), while no significant difference was found between Italian and Swedish nurses in relation to the spot surveys. The nurses questioned directly about WV were significantly younger and reported significantly higher rates of physical aggression (28% vs. 5%, p < 0.001) and all forms of violence (73% vs. 20%, p < 0.001) than those questioned indirectly during the census of all the HCWs. In a multivariate linear regression model, the WV experienced and poor work ability were highly significant predictors of work-related stress (p < 0.001). Nurses who had experienced WV in the previous year had an increased odds ratio (OR = 8.94; Confidence Interval 95% = 4.43; 18.01) of reporting a state of distress. Conclusions: Experience has shown that specific questioning about violence-the commonest method used-encourages respondents to report violent events and may induce overreporting. This method also tends to involve younger workers who are more exposed to WV. On the other hand, prospective studies based on official reports may be influenced by underreporting. Monitoring WV during health promotion interventions included in occupational health surveillance could minimize both phenomena. Systematic studies and meta-analyses which rely mainly on "ad hoc" studies may be biased.

Abstract [en]

To meet the aim of synthesising research on nurses' experiences of promoting personal recovery within psychiatric inpatient care, meta ethnography according to Noblit and Hare has been chosen as the method, which is a qualitative review of scientific literature that results in a synthesis. The focus is on interpretation and is expressed to be able to reduce a story, but at the same time keep what is unique using metaphors. Nine articles formed the basis of the synthesis. The analysis resulted in two levels of synthesis. The first level involved the translation of metaphors into one another, which proceeded in two directions, described as two principal headings. The second level produced five overarching metaphors. The first principal heading is that 'erected walls in psychiatric inpatient care impedes personal recovery.' It encompasses the overarching metaphors 'cultivating in exhausted soil' and 'nurses are gatekeeping watchdogs'. The second principal heading is 'torn-down walls in psychiatric inpatient care enable personal recovery'. It encompasses the overarching metaphors 'disarming and de-escalating', 'facing suffering together reveals treasures' and 'elevating the competent partner promotes recovery'. Conclusively, if the nurse is to be able to promote personal recovery in psychiatric inpatient care, bridges of cooperation and consensus need to be built to other professions in care. Otherwise, the obstacles to promoting personal recovery will be far too hard to overcome.

Abstract [en]

Aim: To explore specialist nurses’ experiences of working with the evidence-based Solihull Approach in Child Healthcare.

Background: In recent decades, changes in living conditions and lifestyles have affected child healthcare, shifting the focus towards children’s mental health. Today’s parents increasingly seek knowledge and information about their children’s emotional needs and development. Research in Child Healthcare indicates a growing need for support in parenting and strengthening the parent-child relationship.

Design and Method: The interview study used a qualitative approach, and a qualitative method was employed for content analysis. The request for participation was sent to a strategic sample of specialist nurses who had participated in the SA training program; the majority (n = 14) were interviewed. The COREQ checklist was used in this study.

Results: The analysis reveals an overarching theme, four categories, and 13 subcategories that illustrate how the training affected the nurses’ attitudes, values, and working methods. They have gained increased awareness of the relationship between parents and children, focusing on facilitating dialogue and individualized care.

Conclusion: The study shows that the Solihull Approach significantly enhances specialist nurses’ competencies in Child Healthcare, resulting in a more person-centred and relationship-oriented approach with parents and children.

Relevance to Clinical Practice: The study revealed that specialist nurses focused more on the impact and quality of interactions between parents and children. Their ability to observe these interactions enabled them to work with parental support. Patient or Public

Contribution: This educational effort enhances how nurses collaborate with parental support, emphasizing the child’s perspective during each visit to Child Healthcare centres.

Abstract [en]

BACKGROUND: The detrimental impacts of adverse childhood experiences (ACEs) on health have been extensively studied, mainly focusing on the effects of ACEs on adults. However, there is a dearth of studies exploring their influences on adolescents.

OBJECTIVE: This study aims to investigate the associations between ACEs and psychological distress among Swedish adolescents.

PARTICIPANTS AND SETTING: At the end of 2020, 1561 Swedish adolescents (15-19-years-old) participated in an electronic survey. The survey encompassed questions about alcohol and drug use problems within the students' families, questions about the students' experiences of physical and psychological abuse, and the administration of the Brief Symptom Inventory to assess their mental health.

METHODS: Descriptive and comparative non-parametric statistics were applied.

RESULTS: Nearly half of the study population reported experiencing one or more ACEs. Exposure to any ACE was significantly associated with a higher level of psychological distress. Of the ACEs examined, the experience of psychological abuse was the most substantial risk factor for psychological distress (with medium effect sizes of ε2 between 0.07 and 0.12 on each domain and a large effect size of ε2 = 0.14 on the General Severity Index scale). There was a positive correlation (r = 0.36) between the number of ACEs experienced and worsened mental health.

CONCLUSIONS: The significant impacts of any ACE, the cumulative number of ACEs, and, particularly, psychological abuse on adolescent mental health are discussed. To further enhance the understanding of these associations and identify effective interventions to prevent and mitigate the negative impact of ACEs on mental health, additional research is warranted.

Abstract [en]

Lifespan in schizophrenia spectrum disorders (SSD) is up to 20 years shorter than average. Introduction of new antipsychotic pharmaceuticals has not made any major improvement. Parallel care is provided as usual standard care (USD) in out-patients settings. The study aim was to evaluate if inclusion of Measurement Based Care (MBC) may prolong patients’ lifespan. In total, 171 patients were followed with annual semi-structured examinations of psychiatric status, social situation, and somatic illnesses over 20 years, by case managers. Findings indicate that MBC contributed to prolonged lifespan in SSD from 55.6 to 69.4 years from beginning to end of the study.
