    Abstract [sv]

    Vid Högskolan Väst i Specialistsjuksköterskeprogrammet som leder till distriktssköterska finns två inriktningar, inriktning mot avancerad vård i hemmet respektive folkhälsa. Utbildningen med inriktning mot avancerad vård i hemmet ger kompetens att arbeta inom kommunal hälso- och sjukvård. Allt fler svårt sjuka människor med många olika sjukdomar och komplexa omvårdnadsbehov vårdas i sina hem. Människor i livets slutskede vårdas alltmer i det egna hemmet omgivna av avancerad medicinteknisk utrustning. I specialistsjuksköterskans arbete ingår att ha förmåga till självständighet, vetenskapligt förhållningssätt, mångkulturellt kunnande och ett professionellt och fördjupat ansvar att leda och utveckla omvårdnaden. I Specialistsjuksköterskeutbildningen vid Högskolan Väst uppnår sjuksköterskorna två examina efter fullgjord utbildning, en yrkesexamen, i förevarande fall distriktssköterska och en generell examen, magisterexamen. Högskolan Väst har anpassat utbildningsbehovet efter hur omvärlden förändrats för att tillmötesgå kompetens inom specialistsjuksköterskeutbildning med fokus på avancerad vård i hemmet.

    Abstract [en]

    Background: Studies showed differences in attitudes towards patients with mental illness in nurses who worked in somatic care compared to nurses who worked in psychiatric care. The nurses in somatic care stated more negative attitudes to mental illness in relation to the nurses in psychiatric care. Studies also showed that work experience affected attitudes towards mental illness. Nurses who often came in contact with people with mental illness had less negative attitudes and prejudices against mental illness. Aim: The aim was to examine the experiences from nurses in somatic care, caring for patients who also have a psychiatric diagnosis. Method: The study was conducted as a semi-structured interview. The study was conducted through interviews with six nurses from the departments with a focus on somatic care. Qualitative content analysis was used for the analysis, which resulted in 48 pieces of sub-categories and six categories. Results: The analysis resulted in six categories: “often psychiatric patients in the somatic care”, “difficult patients”, “sees the whole patient as a person, not a diagnose”, “if it is not written, it does not exist”, “I know how to do, but” and “psychiatric disorder, not for real”. Conclusion: Nurses experienced that caring for patients with psychiatric comorbidity was caring for patients who required a lot of time and attention. The nurses felt they lacked some knowledge how they should respond to, and how to provide the best care, for these patients. Nurse’s experience was that they did not get the support from the psychiatric clinic that they wanted.

    Abstract [en]

    Background: Sexual abuse can be characterized by an adult or a significantly older child using the child's dependency which violates the child's self-esteem. Signs of sexual abuse can be difficult to interpret and reveal itself as physical or psychological symptoms. Health visits gives the school nurse an opportunity to identify children with physical and mental illness. Collaboration and cooperation with social services creates opportunities for the school nurse to manage the suspicion and process of reporting sexual assault. To enable better collaboration social services should provide feedback to the school nurse after a report has been made. Aim: The aim of this study is to describe school nurses' experiences in meeting pupils who have been sexually abused. Method: Two focus group interviews were performed with school nurses who worked in primary and secondary schools. Data from the interviews was analysed with an inductive qualitative content analysis. Results: In the analysis, three categories were found; becoming aware through consents of sexual abuse and through interpretation of signals of sexual abuse, strengths and weaknesses as a school nurse given the opportunities and constraints in meeting with vulnerable pupils and to enhance students' self-esteem by submitting strength. These together will contribute to early identification of pupils who have been sexually abused and give them the support they deserve. Conclusions: School nurses have knowledge of signals which should lead to suspicion of sexual abuse, but feel insecure how to deal with these suspicions. To receive guidance in their work and support in their suspicions from other professions, would provide a sence of security and strengthen them in meeting these pupils. A lack of support lead to prolonged psychological distress . The school nurses want more knowledge about sexual abuse to enhance their skills and to empower vulnerable pupils' self-esteem.

    Abstract [en]

    Background: A Child Health Nurse has great potential to detect and identify children who are at risk for developing overweight and obesity. Overweight and obesity is a contributing cause of ill health later in life. Therefore, prevention and health promotion in life´s early stages is of great importance in order to help prevent obesity.

    Aim: The aim of this study was to describe the Child Health Nurses' experiences and lessons learned of talking to parents about their child's well being in regards to being overweight or obese.

    Method: An inductive and qualitative interview study was conducted. Ten pediatric clinical nurses, all working within the field of Child Health Care in the region of Västra Götaland in Sweden, were interviewed. These interviews were then analyzed using a qualitative content analysis method.

    Results: This study resulted in a conclusion that can be divided into five categories, each important factors that needs to be considered when dealing and preventing obesity at an early age: to choose pedagogical strategies, to ensure the family's situation, to support healthy eating, to be tactful and gentle, and to focus the thinking on the prevention of obesity.

    Conclusion: The Child Health Nurses' are experiencing difficulties when talking with parents about their child being overweight or obese. A good relationship, between the Child Health Nurses and the parents, facilitates the communication, which makes it easier to converse. Another obstacle when addressing these issues with parents is the child's presence during the conversations. Also, it is of great importance to take into account the families' situation as well as potential external factors that can have an impact on their life situation.

    Background

    Domestic violence among the elderly is a worldwide public health problem. It is not noticed in society to the extent that violence against women or children. With an increasing elderly population in the world, it is important that the problem is highlighted and that society considers strategies to help and support vulnerable older people. As the district nurses in their profession faces many elderly patients they have an important role in identifying and dealing with domestic violence among the elderly.

    Aim

    The purpose of this study was to describe the district nurses' perceptions, experiences and attitudes to domestic violence among the elderly.

    Method

    A qualitative approach was used and ten district nurses were interviewed. Respondents were evenly divided between primary care and home care. A qualitative content analysis was conducted by interview material.

    Results

    The analysis resulted in three categories, and each category, three subcategories. The categories that emerged were: Dare to see. The subject was perceived as so complex to handle that many chose not to act despite the suspicion occurred about violence. Dealing with difficult emotions. District nurses experienced many difficult feelings of insecurity, fear and powerlessness. Lack of preparedness. District nurses experienced a lack of knowledge on the subject. There was both emotional and practical barriers. They described that there was no written down, finished strategies, to be able to make use of.

    Conclusions

    The emotions aroused perceived as problematic and brought ethical dilemmas. The lack of knowledge and lack of guidelines contributed to that district nurses waited to act despite indications of violence

    Boken Om kommunikation – för personal i vårdande yrken, tar upp kommunikation såväl mellan enskilda individer som inom grupper och organisationer och mellan aktörer på samhällsnivå. Boken vänder sig till den som utbildar eller arbetar inom ett vårdande yrke, till exempel inom vård, omsorg, socialtjänst, skola och utbildning, personal i olika boendeformer, boendestödjare och kontaktpersoner. Förhoppningen är att boken ska ge inspiration att fundera, reflektera och diskutera om kommunikation samt inspirera till att söka sig vidare för att fördjupa sig.

    Kapitel varvas med intervjuer med personer inom skilda yrken som resonerar kring kommunikation ur sina olika perspektiv. Här finns också ett kapitel om skrivande som utgår från att personal i vårdande yrken ska skriva många olika typer av texter som ska förstås av olika människor med olika förutsättningar. Det gäller att själv utveckla sig som en skrivande person och i kapitlet ges tips och råd kring detta.

    Introduktion:

    Den statliga regleringen av apoteksmonopolet medförde att tillgängligheten av receptfria analgetika blev större men det kan vara svårt att få information om läkemedlen när de försäljs i matvaruaffärer och på bensinstationer. Det har framkommit att felaktig användning av receptfria analgetika kan leda till allvarliga skador.

    Syfte:

    Syftet med föreliggande studie var att undersöka människors erfarenheter och användning av receptfri analgetika.

    Metod:

    En kvalitativ metod användes där människors subjektiva upplevelser beskrivs i berättande form. För att erhålla data gjordes tolv enskilda intervjuer med sju kvinnor och fem män i ålder mellan 24 och 80 år. Därefter gjordes en kvalitativ innehållsanalys av det insamlade transkriberade materialet (Graneheim & Lundman, 2004).

    Resultat:

    Analysen resulterade i tre kategorier och elva subkategorier som beskrev människors erfarenhet och användning av receptfri analgetika i egenvårdssyfte. Olika användningsaspekter: Används för olika syften, Slentrianmässig användning, Mindre farligt med receptfria läkemedel, Inte fått information om interaktioner och Tror inte generika är likvärdiga. Varierad tilltro till information: Vet var de kan hämta information, Otillräcklig information vid inköp, Massmedial information och Råd av vänner. God tillgänglighet: Lätta att få tag på och Bra vid egenvård.

    Slutsats:

    Studien visade att människor inte tog receptfri analgetika enbart i smärtstillande och febernedsättande syfte utan ibland för att må bättre. Det saknades kunskaper om doseringar, biverkningar, interaktioner och generika. De flesta upplevde att de ville få information av utbildad personal och flera deltagare ville därför köpa sin medicin på apoteket. Det som upplevdes positivt var att medicinen var lättillgänglig och att det sparade tid och sjukvårdsresurser. Negativa synpunkter var att medicineringen kunde vara riskfylld. Studieresultatet indikerar att det är viktigt för hälso- och sjukvårdspersonal att ta reda på vad människor känner till och informera om receptfria analgetika vid egenvårdsrådgivning.

    Healthcare professionals work by laws and policies aimed to improve the safety for the patients. This involve working with safety based on a quality aspect there several participants collaborate to develop various assessment instruments to increase patient safety. Safety is a concept with multiple dimensions and there are both internal and external aspects in the concept

    . The internal safety is characterized by the feeling to feel safe which is a combination of balance, peace and harmony. The external safety is characterized by the relationship with the outside world, material things and relationships.

    The purpose of this paper was to describe and deepen the understanding of concept safety in nursing.

    The analysis was conducted using Peep Koorts model for concept analysis. This concept analysis has been worked out in four steps: etymological-, semantic-, discriminations analysis and an interpretation phase. In this analysis there was used twelve Swedish dictionaries, published between year 1981-2014.

    The result showed that it´s unclear what was meant by safety in care

    . The concept safety has many meanings and what is put in the concept is individual by experience, age and education.

    As healthcare professionals, it´s important to be aware that safety can be of a different interpretation, significance and meaning for the patient. In order for the nurse to be able to meet the patient's expectations and needs while following laws and ethical agreements the nurse must be perceptive to the patient's perspective and the subjectivity of the concept safety.

    Abstract [en]

    In this article the authors present a follow-up study of women's interview narratives about life 5 to 7 years after a breast cancer operation. The women had taken part in a study during the 6-month postoperation period. Art therapy contributed to well-being, including strengthening personal boundaries. In the new study, interview analysis informed by critical discursive psychology indicated three problematic discourses that the women still struggled with several years after the operation: the female survivor, the “good woman,” and individual responsibility. We concluded that many women with a history of breast cancer need support several years after their medical treatment is finished.

    Background Chronic obstructive lung disease is a disease that is common among the smoking population. In Sweden, more women than men are smokers. The most effective treatment to improve the symptoms of COPD is to quit smoking but still many women continue to smoke, despite their illness. Aim The aim of this study was to describe how a group of smoking women with chronic obstructive lung disease (COPD) experienced their everyday life and their relationship to smoking. Method A qualitative study based on narratives from six women with COPD was conducted. The narratives focused on the women′s everyday life and their relationship to smoking. The interviews were analysed into four themes and a new story, based on these themes were constructed. Findings The new story describes a woman with COPD that knows what she must do but cannot find the power within herself to take action. She talks about herself like a young bird that is going fly for the first time. Conclusion The study indicates that these women need some form of individual help to find the inner strength that they lack. Their self-respect as well as their self-image needs to be boosted in order to strengthen their belief that they are going to cope without the cigarette.

    Abstract [en]

    Background:

    Overweight and childhood obesity is an ongoing problem in Sweden today. Child health care nurses (CHC nurses) plays a central role in establishing healthy eating habits in children. The importance of introducing healthy eating habits early in life in order to prevent childhood overweight and obesity is well stated.

    Aim:

    The aim was to illuminate CHC nurses´ experiences of establishing good eating habits in children 0-6 years.

    Method:

    Data were collected through semi-structured interviews with nine nurses from Child health care centers in western Sweden. Qualitative content analysis was used to analyze data and resulted in three categories and four subcategories.

    Results:

    The analysis resulted in three categories which represented the CHC nurses work in supporting healthy diets; the professional relationship, emotional experiences and establishing healthy diets. The results show that establishing healthy eating habits in children were dependent on the relationship between the parents and the CHC nurse. The CHC nurse professional experiences were helpful when approaching families, although the feeling of discomfort when addressing unhealthy eating habits remained.

    Conclusion:

    The CHC nurses stated that it was fun and easy to talk to children and their parents about food and healthy diets, as long as they had the same approach. When the parents had a different opinion it became a sensitive and difficult task. The CHC nurses struggled with targeted actions preventing unhealthy eating habits.

    Aim: To elucidate nurses’ (RNs) experiences of co-ordinated care planning in outpatient care. Background: Co-ordinated care planning has been studied from the perspectives of both patients and nurses in inpatient care, but it is missing in outpatient care.

    Method: Qualitative content analysis of interviews with ten registered nurses participating in two focus groups.

    Results: An overall theme was identified: creating concordant communication in relation to patient and health-care providers. The result is based on four categories and nine sub-categories.

    Conclusions: Nurses need extraordinary communication skills to reach concordance in outpatient care planning. Apart from involving and supporting the patients and next of kin in the decision-making process, the outcome of the nursing process must be understood by colleagues and members of other professions and health-care providers (non-nursing).

    Implications for nursing management: An effective outpatient care-planning process requires that care managers understand the impact of communicating, transferring information and reaching agreement with other health-care providers, actively supporting employees in the outpatient care-planning process and contributing to the development of common goals and policy documents across organisational boundaries.

    Background: Eating-related distress describes as common symptoms in patients with advanced cancer in a palliative stage. Different symptoms such as nausea, vomiting, anorexia and kakexi can exist and these symptoms causes different experiences to patients. Some of the patients have ongoing chemotherapy treatment. Swanson's theory of caring describes how nurses can support a person trough difficult situations and transitions in life and contains five caring processes.

    Aim: To throw lights upon experiences of eating-related distress among patients with cancer in a palliative stage

    Method: A qualitative study with interviews of eight patients and data analysis based on content analysis

    Results: The findings describes two unit of analysis, the experiences of eating-related distress with categories and subcategories; Affected appetite - anorexia and drug effects, Affected bodily function - weight change, function of stoma, physical ability and nausea together with vomiting, Emotional impact - forcing one to eat, psychological ability and social community. The next unit of analysis was need of support; The need for information and the need for security - through the nurse, through next of kin and through other patients

    Conclusions: When eating doesn't work the patient is affected in many different ways, both physical and psychological. The patients also need information and different forms of support from the nurse, next of kin and other patients.

    Abstract [en]

    Background Previous research studies have found that the better the quality of practice environments in hospitals, the better the outcomes for nurses and patients. Practice environment may influence nurses’ ability to individualize care but the detailed relationship between individualized care and the professional practice environment has not been investigated widely. Some evidence exists about the association of practice environments with the level of individualization of nursing care, but this evidence is based on single national studies. Objectives The aim of this study was to determine whether nurses’ views of their professional practice environment associate with their views of the level of care individualization in seven countries. Design This study had an international, multisite, prospective, cross-sectional, exploratory survey design. Settings The study involved acute orthopedic and trauma surgical inpatient wards (n = 91) in acute care hospitals (n = 34) in seven countries, Cyprus, Finland, Greece, the State of Kansas, USA, Portugal, Sweden, and Turkey. Participants Nurses (n = 1163), registered or licensed practical, working in direct patient care, in orthopedic and trauma inpatient units in acute care hospitals in seven countries participated in the study. Methods Self-administered questionnaires, including two instruments, the Revised Professional Practice Environment and the Individualized Care Scale-Nurse (Individualized Care Scale-Nurse A and B) were used for data collection. Data were analyzed statistically using descriptive statistics, simultaneous multiple regression analysis, and generalized linear model. Results Two regression models were applied to assess the predictive validity of the Revised Professional Practice Environment on the Individualized Care Scale-Nurse-A and B. The results showed that elements of the professional practice environment were associated with care individualization. Internal work motivation, cultural sensitivity, control over practice, teamwork, and staff relationship with physicians were predictors of support (Individualized Care Scale-A) for and the delivery (Individualized Care Scale-B) of individualized care. Conclusions The results of this study provide evidence that environment aspect could explain variations in care individualization. These findings support the assertion that individualized care needs to be understood in a broader context than the immediate nurse–patient relationship and that careful development of the care environment may be an effective way to improve care quality and outcomes.

    In previous research, no uniform picture emerged of the role of relatives in the meeting between an elderly patient and a physician. Knowledge about relatives' experiences of the meeting between an elderly patient and a physician will help healthcare practitioners better understand the role of relatives during the meeting and how practitioners can assist relatives in assuming their supporting role more efficiently. The purpose of this study is to explore experiences of relatives of meeting with the physician in a hospital setting when an elderly patient is discharged from hospital care to home care, in order to identify aspects that may facilitate relatives in taking up their role in a more efficient manner. This descriptive and exploratory study is based on 20 interviews with relatives. The result shows that the physician's communication style influences the meeting between the relative, the elderly patient, and the physician, and that this style is the result of power and interaction. A trustful relationship during the meeting between the relative and the physician can increase the relative's feeling of confidence with the healthcare organization and treatment of the elderly patient. The relative has an important supporting role in the care for the elderly family member, both in the hospital and the home setting. It is likely that the relative's value as a resource, for both the patient and the physician, increases as the relative experiences feelings of confidence in the meeting with the physician. It is therefore of value to increase our knowledge about the conditions and circumstances facilitating and/or hampering the meeting between the relative and the physician. The result stresses the importance of encouraging relatives to participate in the meeting. Physicians need more guidance and training in communication skills, respectful demeanor, and collaboration while meeting the relatives. © 2013 S. Pennbrant.

    Abstract [en]

    The aim of this study was to clarify the meanings and dimensions of the concepts “profession” and “role.” The results from the concept determination were discussed in relation to the profession “nurse educator.” This study is based on Koort's semantic analysis methods, using select parts of Eriksson's approach for concept determination, using dictionaries published between the years 1948 and 2015. The findings underline the complexity of the professional role of nurse educators. The nurse educator profession is based on society's trust and requires integration of ability, attitudes, norms, reflection, and theoretical knowledge, along with individual, organizational, and social conditions. Nurse educators must achieve a sufficient degree of pedagogical competence, subject competence, social competence and organizational competence in order to develop their professional role. When nurse educators define their function, a professional role takes form.

    Abstract [en]

    Previous research has provided contradictory findings on how important it is for elderly patients to actively participate in the meeting with their doctors. Using descriptive and exploratory interview study with 20 elderly patients discharged from medicine and geriatric hospital care in Sweden, the authors describe how elderly patients experience their meetings with their doctor in the hospital setting. The results indicate that the meetings between elderly patients and doctors are influenced by the nature and shape of the conversation, which are influenced by power and interaction. A good relationship between an elderly patient and his or her doctor leads to reduced apprehension and increased faith in the health care system. This study was inspired by the sociocultural perspective and highlights the importance of the health care sector becoming a learning organization in which doctors are trained to prevent misunderstandings in their meetings with elderly patients.

    Abstract [en]

    Professional development is a process starting during undergraduate education and continuing throughout working life. A new nurse's transition from school to work has been described as difficult. This study aims to develop a model describing the professional development of new nurses during their first years of work. To develop this model, constant comparative analyses were performed. The method was a qualitative study of survey data on 330 registered nurses. The results showed that mastering the professional role was the result of an ongoing process building on the nurse's experiences and interactions with the surrounding environment. The professional developmental process involves the following interrelated sub-processes: evaluating and re-evaluating educational experiences, developing professional self-efficacy and developing clinical competence. These sub-processes are influenced by the following factors: social values and norms, healthcare organization, management of new nurses, co-workers, patients and significant others and the nurse's own family and friends. These factors affect professional development directly, indirectly or as mediating influences and can lead to possible outcomes, as new nurses choose to remain in or leave the profession. The results underscore the importance of developing a professional nursing role within the new working context. To facilitate this professional development, new nurses need support from their nursing-school educators and their healthcare employers. The model described here will be the subject of further measurement and testing. © 2012 Elsevier Ltd.

    Objective: The aim of this study was to describe nurses’ experiences of caring for palliative-stage patients in a hospital setting.

    Methods: A qualitative study based on six interviews with strategically selected nurses in an oncology department in western Sweden. The interviews were analyzed using a phenomenological-hermeneutic approach.

    Results: The results identified three themes: 1) Developing professional self-confidence; 2) Developing communication skills; and 3) Having time for reflection. Each theme has at least two subthemes.

    Conclusions: Caring for palliative-stage patients requires a combination of knowledge, experience and professional and personal self-confidence. Nurses need regular coaching by staff and professionals with more experience dealing with and processing suffering, death and related existential issues. Nurses need to separate work from private life and have satisfying and relaxing leisure activities.

    Smoking is a public health problem and can create lack of health in the general population. The nurse´s work includes promoting health. Helping and supporting patients to lifestyle changes, such as smoking cessation, is one way for the nurse to promote health.

    Aim

    The aim of the study was to investigate how the nurse promotes health and supports patients in smoking cessation.

    Method

    A questionnaire was sent to 114 nurses working with smoking cessation in western part of Sweden. The questionnaire contained both closed and open questions. The answers to the closed questions were analyzed using descriptive statistics and the open questions were analyzed by content analysis.

    Results

    The nurses' work includes motivating, educating, supporting, inform and follow up patients in need of smoking cessation. Many nurses, 63%, consider themselves to have an important function in smoking cessation. Lack of time is a noticeable problem, all nurses of the study spend 1-10 hours/week to smoking cessation. Nearly half of the nurses, 43%, felt that they didn´t had enough knowledge in smoking cessation. The nurses wish to learn from other nurses experiences.

    Conclusion

    Nurses experience lack of time and knowledge about smoking cessation. In order to provide smoking cessation of good quality the nurse should possess medical knowledge and be empathic and involved. Research about smoking cessation are not discussed much in the nurses working places.

    Aim: Investigate how the district nurse promotes health and supports patients in smoking cessation.

    Method: A questionnaire was sent to 124 district nurses working with smoking cessation in a region in western Sweden. The questionnaire contained both closed and open questions. The responses were analyzed using descriptive statistics and manifest content analysis.

    Findings: The district nurses’ work includes motivating, educating, supporting, informing and following up patients in need of smoking cessation. Sixty-three percent of the district nurses consider themselves to have an important function in smoking cessation. Lack of time is a substantial problem and district nurses devote one to ten hours per week on smoking cessation. Forty-three percent felt that they did not have enough knowledge about smoking cessation. The district nurses wished to benefit from the experience of other colleagues.

    Conclusion: District nurses experience lack of time and knowledge. In order to provide quality support for smoking cessation district nurses should possess medical knowledge and be empathic and involved.

    In recent years there has been increased intimate partner violence (IPV) toward women. Research on the care provided to victims of IPV is limited. The purpose of the study was to gain a deeper understanding of women's lived experience of IPV and their encounters with healthcare professionals, social workers, and the police following IPV. A phenomenological hermeneutic method inspired by the philosophy of Paul Ricoeur was used. The method is based on text interpretation and gives voice to women's lived experience. Twelve women living at a women's shelter in Sweden narrated their IPV experiences. The study revealed that the women experienced retraumatization, uncaring behaviors, and unendurable suffering during their encounter with healthcare professionals. They were disappointed, dismayed, and saddened by the lack of support, care, and empathy. Nurses and other healthcare professionals must understand and detect signs of IPV as well as provide adequate care, as these women are vulnerable. IPV victims need to feel that they can trust healthcare professionals. Lack of trust can lead to less women reporting IPV and seeking help.

    Background: Effective health care requires good communication between the nurses and the patients. Being able to understand patients and to make them understand you are very important to provide the best care to the patients. Health care meeting of elderly patients who are not fluent in Swedish language with nurses who do not speak their language leads to miscommunication which hinders exchange of health care information. The aim of present study was to describe experiences of old Iranian patients receiving care from Iranian nurses in health care encounter. Method: This study used a qualitative approach. For data collection, we used qualitative interviews with open-ended questions. The nine interviews were analyzed by means of content analysis. The result showed that these patients were positive and satisfied with the care that received from Iranian nurses. Being able to communicate and discuss directly with the nurses, patients felt satisfied and would prefer to meet Iranian nurses again when they have health problems. Health care encounters were regarded as inconvenient and time-consuming when the patients were in need of someone else's interpretation. Most participants in this study felt that they met with openness and courtesy of Iranian nurses, which created trust among the patients. Intercultural awareness created a feeling of security and belonging among the patients. Stress and lack of time seemed to be two of the reasons for the negative attitudes and response of Iranian nurses.

    Introduction: Violence and intimidation by patients against staff in the health care units is a major problem, but despite this, it is difficult to find literature that illustrates the problem. Purpose: The aim was to describe the nurses' experience of violence in her/his workplace with patients with mental illness. Method: This qualitative interview study was, analyzed inductively using qualitative content analysis. Results: The result shows that the nurses have different experience of threats and violence within the health care. Four categories emerged; difficult to do ones work, nurses play down, threat create fear, the feeling of frustration and eight subcategories. These, ended up in the theme meeting as a tool and possibility. The fear and uncertainty that some nurses experienced as the biggest barrier to reach the patients, since these feelings can cause attitude towards the patients that the nurses will carry with them long after. The nurse then feels frustrated that she/he doesn't know what the reason is for the mentally ill patient to be so angry. Conclusion: The conclusion of the study Threats and Violence in healthcare generally creates an insecure working environment for nurses and can prevent a professional approach. Some nurses have internal ability to handle the situation with violent patients, while other nurses wish more training about the treatments and communications. Training about how mutual trust can be created to help the nurse identify a violent behavior and prevent this in timely manner before the encounter turns into a threatening situation.

    Bakgrund:

    Enligt kompetensbeskrivningen ska distriktssköterskan ge vård utifrån ett folkhälsoperspektiv, och därmed arbeta hälsofrämjande gentemot större patientgrupper samt befolkningen som helhet. Tidigare forskning visar emellertid behovet av ökad förståelse för distriktssköterskans hälsofrämjande arbete.

    Syfte:

    Att belysa distriktssköterskors erfarenheter av hälsofrämjande arbete inom primärvården.

    Metod:

    Studien hade en kvalitativ ansats där fyra distriktssköterskor och en sjuksköterska intervjuats med hjälp av en semistrukturerad intervjuguide. Kvalitativ innehållsanalys användes som analysmetod.

    Resultat:

    Från analysen framkom följande kategorier: Att hälsoarbetet förändrats över tid, Att sätta patienten i centrum, Att uppleva brist på tid för hälsofrämjande arbete, Att få tid till hälsofrämjande arbete, Att styras av ekonomisk prioritering, Att få möjlighet till kompetensutveckling – en förutsättning, Att tidigt vilja förebygga ohälsa, Att få stöd till hälsofrämjande arbete.

    Slutsats:

    Behovet av en ökad förståelse för en högre prioritering av förebyggande hälsoarbete för att kunna hantera framtidens sjukvårdsbehov. Primärvården är alltför medicinskt inriktad och behöver istället fokusera mer på hälsa. Det är därför viktigt att arbetsgivare i primärvården kan ta del av studiens resultat för att främja bättre arbetsförutsättningar för distriktssköterskors hälsofrämjande vård.

    Personer med psykiska funktionshinder drabbas i stor utsträckning av åkommor som kräver medicinsk vård. En stor del av denna patientgrupp vårdas av kommunal vård och omsorg. Studier visar att sjuksköterskan i den somatiska vården, upplever svårigheter med att möta och vårda patienter med psykiska funktionshinder samt att patienterna inte alltid får den vård de behöver. Sjuksköterskan har en viktig roll i att möta och tillgodose behov som kan uppstå i vården av personer med psykiska funktionshinder samt ge god vård på lika villkor. Syfte: Syftet med studien var att belysa sjuksköterskors erfarenheter av att möta personer med psykiska funktionshinder i hemsjukvård. Metod: Sjuksköterskor från hemsjukvården med minst två års arbetserfarenhet tillfrågades att delta i studien. En intervjustudie genomfördes för att fånga och belysa sjuksköterskors erfarenheter. Kvalitativ innehållsanalys användes för att analysera intervjutexterna. Resultat: Analysen resulterade i fem kategorier. Dessa är vilja att skapa trygghet och tillgänglighet, betydelsen av att skapa en dialog, vilja att stödja till insikt och självbestämmande, att ta stöd i riktlinjer och expertkunnande för att bedriva god vård samt att hantera egna fördomar, förutfattade meningar och bristande kompetens. Konklusion: Resultatet visar på betydelsen av att sjuksköterskor har tillräcklig kunskap och erfarenhet för att möta och tillgodose behov som kan uppstå i vården av personer med psykiska funktionshinder. Psykiatrisjuksköterskans specialistkompetens skulle kunna tillvaratas för att utbilda och handleda sjuksköterskor i hemsjukvård. Ökad kunskap kan medföra att sjuksköterskor känner sig mer säkra i mötet och att de på ett professionellt sätt kan möta och vårda denna patientgrupp.

    Abstract [en]

    The present paper focuses on the process of understanding the patient in the context of perioperative caring and reports a story narrated by a perioperative nurse as well as her emerging understanding of the patient prior to surgery at an operating department. This qualitative case study had a dual purpose; firstly, to describe how the perioperative nurse's understanding of the patient emerged and, secondly, to establish how the researcher interpreted the situation. As a perioperative nurse and researcher, the author is both the narrator and interpreter. To date we have rarely discussed the fact that, in a perioperative context, the patient might feel ashamed of his/her body, even before arriving at the operating department. This new understanding emerged from the hermeneutical dialogue in the present study.

    Aim

    To describe nurse anaesthetists and theatre nurses experiences of working with the perioperative dialogue.

    Background

    A new way of working in the perioperative context is the perioperative dialogue model, which is the meeting between the nurse and the patient before, during and after a patient’s surgery.

    Method

    Semi-structured interviews were conducted with 20 nurses, trained in working with the perioperative dialogue. Data were analysed according to grounded theory.

    Findings

    One core category, source of strength, and two main categories, walking together and creating a caring relation, were defined. These represented factors in developing nurses work and professional competence. The nurses became more committed to their work when they came to know the person beyond the character of the patient.

    Conclusions

    The perioperative dialogue is a source of strength to the nurses. It is necessary that the organization supports nurses in this new way of working.

    Abstract [en]

    Background. This research was based on previous qualitative research consisting of five sub studies, in which a hermeneutic approach was employed to identify how caring becomes visible in clinical praxis and how it is carried out in the context of a Swedish research programme.

    The aim of the research was to explore the ethical foundations of caring in the perioperative dialogue and culture on the basis of previous research and philosophical literature, as well as to focus on ethos as a vital part in the common world of the perioperative dialogue.

    Methodology. The research employed a hermeneutic approach grounded in the perspective of the humanistic caring tradition of caring science, where ethos confers the ultimate meaning to the caring context.

    Results. This approach resulted in a theoretical model comprising the caring properties and characteristic caring principles of the perioperative dialogue structure. As a guiding principle for the perioperative common world, ethos revealed a deeper principle of structure; the responsibility of the face – “here I am”. The inner core of caring can be approached through communion, which forms a continuous whole in the patient’s and nurse’s common world and leads to health for the participants.

    Spirituality is often mistakenly equated with religion but is in fact a far broader concept. The aim of this integrative review was to describe experiences of the positive impact of spirituality and spiritual values in the context of nursing. The analysis was guided by Whittemore and Knafl’s integrative review method. The findings revealed seven themes: ‘Being part of a greater wholeness’, ‘Togetherness − value based relationships’, ‘Developing inner strength’, ‘Ministering to patients’, ‘Maintaining one’s sense of humanity’, ‘Viewing life as a gift evokes a desire to ‘give back’’ and ‘Achieving closure − life goes on’. It is difficult to draw definite conclusions, as spirituality involves many perspectives on various levels of awareness. However, spirituality was considered more inclusive, fluid and personal. Furthermore, it emerged that spirituality and spiritual values in the context of nursing are closely intertwined with the concept of caring.

    Aim:

    To illuminate nurses' experiences of caring for sick children community home healthcare.

    Background:

    There is an international consensus that children with different types of medical

    conditions, to an extent as far as possible, should receive care in their homes. Home healthcare allows the family to stay together when the child is undergoing treatment and thus reduces the strain on the family. Children as a patient group is a relatively new phenomenon in community home healthcare in Sweden and represents a fairly small part of the community nurse responsibilities, which make it difficult to accumulate experience and practice.

    Design:

    The study used a qualitative, descriptive design.

    Methods:

    Individual interviews were conducted with twelve nurses working in community healthcare

    in the western parts of Sweden. Qualitative content analysis was used to analyse the data.

    Results:

    One overall theme, "to feel confident in order to create confidence", two main themes,"to

    build a trusting relationship with the family" and "to feel confident in the role as caregivers", emerged

    together with six subthemes. The nurses aimed at creating a trusting nursing relationship and

    working closely with the parents. The feeling of confidence in their role as caregivers was closely connected to the level of support and knowledge the nurses received.

    Conclusion:

    Many nurses felt unsecure due to the task of caring for children in community home

    healthcare. However, most nurses experienced the situation as manageable with peer support and distinct collaboration with the hospital. Guidance and education was called for in order to experience greater confidence in the professional role as caregivers.

    Relevance to clinical practice:

    The implication of the study may contribute to efforts being made to extend and improve cooperation between remitting pediatric clinic and community healthcare

    Background

    The primal duty for a district nurse is treating all persons fairly and equitably. Unfortunately you could see through previous research that there is injustice in health care. Some of acts are done consciously and some of them unconsciously.

    Aim

    The aim was to illuminate the district nurses' experience of equal care.

    Method

    Data was collected through semi structured interviews and has a qualitative approach. The data material was analyzed through qualitative content analysis.

    Results

    The three categories that emerged from the outcome was: To be in a certain way with four sub-categories: that personality influences, that relatives influence, that those who require it get it, and belonging to a vulnerable patient population. To be seen with two sub-categories to make different and to be equal. To be in the right place with two subcategories to get professional treatment and that the health care context is important.

    Conclusion:

    Discussion took place regarding the need to set aside more time to discuss issues of equality in the workplace. To set time aside for reflection on the district nurse's own approach would probably reduce injustice in health care.

    Background: The need for structured practice has in recent years appeared in connection with the request for a more evidence-based nursing. In psychiatric care with its complex disabilities, it is important to have methods to identify these. Research has demonstrated the importance of patient participation in nursing that has been shown to affect their mental health. It is essential to get to know how patients with mental illness are capable of involvement in their life situation. The ICF emerged as a useful theoretical framework for development of questionnaires of patient participation.

    Aim: The aim of the present pilotstudy was to develop and test a questionnaire for participation, based on ICF. Method: This pilotstudy had a quantitative approach and was based on a draft for the purpose described. For the development process two expertgroups were used, one consisted of nurses with experience of ICF and one by former patients recruited from psychiatric patient organizations to discuss participation and item selection. The final questionnaire was answered by 29 nurses, who also could comment on the content in three open questions used for face validity. Cronbach´s alpha was used to calculate internal consistency. The final goal is to develop a concept of two questionnaires for participation. One that will be answered by nurses and another part based on patients self-assessment. During this pilotstudy, the nurse`s part was focused.

    Result: The result indicated good internal consistency both in the whole Assessment of Participation in Psychiatric care (APP) (0.900) and for each item (0.892-0.904). We also found indication for good face validity.  The result of this pilot study can be useful for further research and development.

    Introduction: Medical student's professional attitudes are expected to be developed in medical school, and particularly during clinical education. In this study we focus on supervision in order to describe the attitudes emphasized in the clinical education of fourth-year medical students taking a surgical course.Methods: An ethnographic approach was applied where observation and interviews were conducted. Nine medical students and twelve supervisors at a teaching hospital in Sweden participated. Field notes were made during observation as well as interviews; these were transcribed and analysed qualitatively.Result: The analysis resulted in six topic areas describing the attitudes emphasized. The medical students were expected to be: 1) Informed and effective decision makers, 2) Sensitive to patients' needs and expectations, 3) Communicative, 4) Authoritative and patriarchal, 5) Adaptable to organizational demands, and 6) Mindful of nurse's knowledge and requests.Conclusions: This study reveals that the attitudes emphasised during supervision are: dualistic and complex to learn, developed by a former generation and influence student learning. Students need support in order to handle the state of tension that exists in the attitudes emphasized. Medical students might experience difficulties in adopting some attitudes belonging to a former generation. There is a need for competence development among supervisors concerning how students may experience the attitudes emphasized in supervision.

    Background:  Clinical teaching is a complex learning situation influenced by the learning content, the setting and the participants' actions and interactions. Few empirical studies have been conducted in order to explore how clinical supervision is carried out in authentic situations. In this study we explore how clinical teaching is carried out in a clinical environment with medical students.

    Methods:  Following an ethnographic approach looking for meaning patterns, similarities and differences in how clinical teachers manage clinical teaching; non-participant observations and informal interviews were conducted during a four month period 2004-2005. The setting was at a teaching hospital in Sweden. The participants were clinical teachers and their 4th year medical students taking a course in surgery. The observations were guided by the aim of the study. Observational notes and notes from informal interviews were transcribed after each observation and all data material was analysed qualitatively.

    Results:  Seven pedagogical strategies were found to be applied, namely: 1) Questions and answers, 2) Lecturing, 3) Piloting, 4) Prompting, 5) Supplementing, 6) Demonstrating, and 7) Intervening.

    Conclusions:  This study contributes to previous research in describing a repertoire of pedagogical strategies used in clinical education. The findings showed that three superordinate qualitatively different ways of teaching could be identified that fit Ramsden's model. Each of these pedagogical strategies encompass different focus in teaching; either a focus on the teacher's knowledge and behaviour or the student's behaviour and understanding. We suggest that an increased awareness of the strategies in use will increase clinical teachers' teaching skills and the consequences they will have on the students' ability to learn. The pedagogical strategies need to be considered and scrutinized in further research in order to verify their impact on students' learning.

    Background

    Today, parents of children newly diagnosed with type 1 Diabetes (T1DM) mainly learn to weigh and count the carbohydrates in meals to accuratly dose insulin. Previously, parents often dosed insulin after having estimated the amount of carbohydrates only with perceptiveness. It is not uncommon for parents to experience anxiety, and stress contemporaneous with diagnostics. The anxiety is often associated with fear of hypoglycemia. Aim The purpose of this study was to highlight the parents' experience of administrate insulin using carbohydrate counting or eye measurement method in children with T1DM. Method Eight interviews were conducted and analysed using qualitative content analysis. Results The analysis resulted in the theme: The need of strategies, four categories: confidence, stress freedom, uncertainty and as well as seven sub-categories: ease in everyday, ease for others, demanding, to feel controlled, find your own way, groping in the dark and losing control. Most often families used a combination of both methods to find a way that suited them and their situation of life.     Bakgrund: Sjuksköterskan i hemsjukvården har hög arbetsbelastning i förhållande till tid och antal patienter. Vilka förmågor hos henne upplevs skapa förutsättningar för att "fånga stunden" med den äldre trots att mötena många gånger är korta?Syfte: Att tolka och förstå sjuksköterskors upplevelser av att fånga stunden i mötet med den äldre i hemsjukvården.Metod: Hermeneutisk tolkning av intervjuer med sju sjuksköterskor.Resultat: Tre huvudteman framkom vilka var "att bjuda in", "att vara närvarande" och "att bygga broar". "Att bjuda in" innefattade " att förmedla trygghet", " att förmedla förtroende och våga vara nära" och " att vara ödmjuk, lyhörd och tillgänglig". "Att vara närvarande" innefattade " att vara här och nu och lägga allt annat åt sidan", " att känna av, ha tålamod och ta sig tid". "Att bygga broar" innefattade " att skapa harmoni och inte pressa på", " att ha fantasi" och " att ha ett genuint intresse och våga vara uppriktig".Slutsats: Utifrån sjuksköterskans erfarenhet, kunskap och professionalitet möjliggjordes att bjuda in i mötet och skapa en ömsesidig relation. Förmåga att skapa närhet, visa ömhet och vara tillgänglig var då viktigt. Att ta sig tid, hantera stress, ha tålamod och förmedla en gemenskap skapade förutsättningar för samhörighet och förståelse genom att vara närvarande. Att skapa harmoni, våga ha roligt och inte pressa på var ett förhållningssätt som skapade förutsättningar för att bygga broar. Genuint intresse för den äldre och dennes situation och sjuksköterskans förmåga att förmedla sin inre kallelse och glädje ökade möjligheterna till känsla av närvaro i mötet.

    The aim of this study was to investigate the construct validity of the Moral Development Scale for Professionals (MDSP) using structural equation modeling. The instrument is a 12-item self-report instrument, developed in the Scandinavian cultural context and based on Kohlberg's theory. A hypothesized simplex structure model underlying the MDSP was tested through structural equation modeling. Validity was also tested as the proportion of respondents older than 20 years that reached the highest moral level, which according to the theory should be small. A convenience sample of 339 nursing students with a mean age of 25.3 years participated. Results confirmed the simplex model structure, indicating that MDSP reflects a moral construct empirically organized from low to high. A minority of respondents >20 years of age (13.5%) scored more than 80% on the highest moral level. The findings support the construct validity of the MDSP and the stages and levels in Kohlberg's theory

    Background:

    There are national guidelines for cardiac care and the nursing care as outlined in the study. Caring of patients with heart failure consists of seeing the whole picture around the patient. The nurses are responsible for the care and information to the patient with heart failure. Furthermore, the nurse also is responsible for over-reporting among providers to provide heart failure patients continued good care. It is of interest to find out the nurses experiences of caring for patient with heart failure.

    Aim:

    The aim was to describe nurses' experience of caring for patients with heart failure.

    Method:

    Qualitative interview studies with nine nurses were contacted. The analysis resulted in five which led to two categories safety and collaboration.

    Results:

    To create peace for patients with heart failure became an important category, and the results showed a difference in how a nurse creates peace depending on the workplace and education. Second category was collusion in which the results showed that better cooperation is needed to create good nursing care for patients with heart failure.

    Conclusion:

    Nurses sees the creation of giving peace to patients with heart failure as an important part of nursing. Nurses create peace in different ways depending on the workplace and patient needs. All nurses believe that collaboration between different caregivers today malfunction.

    Bakgrund:

    Hemsjukvård är ett område för avancerad vård. Behovet av kvalificerad hemsjukvård ställer stora krav på ökad professionalitet hos hemsjukvårdsjuksköterskan, en kompetens som inkluderar vetenskapligt kunskap och förståelse för att försäkra en kvalitativ och säker vård för patienterna.

    Syfte

    : Syftet med denna studie var att beskriva hur den vetenskapliga skolningen och det vetenskapliga förhållningssättet kommer till uttryck i hemsjukvården.

    Metod:

    Studien fokuserade på specialistsjuksköterskor med avlagd magisterexamen i omvårdnad med arbetsfält inom hemsjukvård. Data erhölls genom telefonintervjuer med åtta specialistsjuksköterskor. Intervjuerna analyserades med hjälp av kvalitativ innehållsanalys.

    Resultat:

    Analysen resulterade i fyra kategorier som beskriver på vilket sätt den vetenskapliga utbildningen förändrat deras sätt att närma sig arbetet. Kategorierna var: förmåga att utveckla sin egen förståelse och kunnande, förmåga att leda och utveckla, att känna professionell trygghet och en förmåga att arbeta utifrån nya tankemönster och perspektiv.

    Konklusion:

    Studien visade att den fördjupade vetenskapliga utbildningen är viktig för sjuksköterskornas professionella utveckling. Utveckling kan ses i specialistsjuksköterskans beskrivningar som avslöjar en kvalitativ förändring i deras sätt att reagera, kommunicera och hantera olika situationer.

    Background:

    Pressure ulcers are a major problem especially for the victim. Suffering in the elderly can be both large and long-lasting pain and impaired quality of life. Most pressure ulcers could be prevented by means of preventive measures. As there are many who suffer poses significant resource requirements and costs of treatment. Aim: To describe the prevalence of pressure ulcers and risk of pressure ulcers among elderly patients, 85+. Method: The study was descriptive with quantitative approach. The study was a survey of pressure ulcers in elderly ≥ 85 years in quality registry Senior Alert.

    The instrument was based on the Modified Norton Scale.

    Results: The study included a total of 10,882 registered assessments. The mean age was 90.3 years. Estimates from the men represented 29.6% and 70.4% of women. There was a statistical difference between the sexes regarding the risk of pressure ulcers. The number of assessments that showed the risk of pressure ulcers was 3347 (31%). 1270 (100%) assessments had been planned actions and follow-up. Conclusion: A third of older patients had a risk of pressure ulcers.

    Abstract [en]

    Aim Swedish Healthcare Direct is an organization staffed by registered nurses who act as telenurses and assess callers’ need for care, taking both medical and personal aspects into account. They direct the care seeker to: emergency care (level I), a care center on duty (level II), their regular doctor (level III), or provide advice about self-care strategies (level IV). In this assessment process, the nurse and care seeker should reach mutual agreement. The aim and focus of the present study was to elucidate the care seeker’s situation and experiences of the care received after being triaged and directed to level II, although the telenurse in fact assessed their medical problems as corresponding to level III. Methods A total of eight recent Swedish Healthcare Direct users were interviewed in this qualitative study. Data were analyzed by means of content analysis. Results Three themes emerged – “feeling trapped”, “feeling disrespected”, and “feeling invited” – comprising seven subthemes. Conclusion Care seekers experienced suffering as well as struggling to be allowed to be a patient. When not met by an ethically correct stance, they continued to seek care. In contrast, when they encountered commitment and an ethically correct attitude, their health process began and they had no further need for contact.

    Abstract [en]

    Background: The number of patients with a foreign background is increasing, thereby it can be a challenge for the staff to communicate with those who do not speak the native language. Therefore, the need for good communication is crucial in order to meet patient’s right and secure the quality of nursing care. Interpreters are increasingly used in healthcare today, despite of this the communication between the nurse and patient does not work. Few studies are made where the communication between the nurse and patient through an interpreter is studied in the primary care in Sweden. Aim: The purpose of this study was to describe nurse’s experience of communication with patients through an interpreter in the primary care.

    Method: Eight nurses and district nurses were interviewed individually during winter 2013. The interviews were recorded and transcribed. A qualitative content analysis method was used to analyse the interview texts. Results: Four main categories were revealed: experiences of communicating through an interpreter, family member as an interpreter, professionalism among interpreters and strategies for the use of interpreters. It is a challenge to work through an interpreter since it require more time and resources. There are differences in the interpreter’s skills and to use relatives as interpreters is not optimal for the patient's autonomy. Nurses have their own strategies when it comes to the use of interpreters. In spite of this it is nurses pointed out the need for training in intercultural communication and training in effective use of interpreters. Conclusion: There is a need for better collaboration between the interpreter agency and the nurses there the views and expectations of both sides can be discussed. That could improve quality of the communication with the patient and provide the care that is planned. Training for nurses in intercultural communication should also be improved
