    Abstract [en]

    This study focuses on 19 juvenile offenders who were frequently intoxicated by flunitrazepam (FZ), almost exclusively under the brand name Rohypnol. Street names for Rohypnol tablets are Rophies, Ropies, Roofies, Ropes, Roches, Rochas, Rochas Dos, Rophs, Ropers, Ribs, R-25, Roach-2s, Trip and Fall, Remember All, Mind Erasers, Forget Pills, and the Date Rape Drug. An overdose of FZ gives an increased feeling of power and self-esteem, reduces fear and insecurity, and provides the belief that everything is possible. FZ is also associated with loss of episodic memory and with impulsive violence, particularly when combined with alcohol. The subjects were taken from a subpopulation of 47 male juvenile offenders from Swedish national correctional institutions. Background information for subjects was obtained by in-depth interviewing and personality inventories including the Zuckerman Sensation-Seeking Scales, the Eysenck Personality Questionnaire, and the Karolinska Scales of Personality. Data concerning previous criminal offenses was obtained from the Swedish National Police Board. Almost all of the FZ abusers had been previously sentenced for serious violent offenses. Our data suggest that FZ abused by psychiatrically vulnerable subjects (i.e., with high scores on boredom susceptibility and verbal aggression) poses a serious hazard both to the abusers as well as the community. Our results support the finding that FZ should be classified as a Schedule I drug (i.e., a drug similar to heavy narcotics).

    Iaktagelser, främst av kriminella män, visar att vissa reagerar med kraftigt våld och förlust av hämningar på överdos av flunitrazepam. Det är troligt att detta beror på sjunkande serotoninnivå i hjärnan. Här beskrivs ett fall med en Rohypnolmissbrukare, dömd för våldsbrott. För att flunitrazepampreparat inte ska förskrivas som vilken annan lugnande medicin som helst, anser artikelförfattarna att dessa ska klassas som tung narkotika, som i USA.

    Background: Long-term stress can lead to reduced health among nurses which can result in impaired quality of care. Stress in nurses' work needs to be studied in order to prevent stress and impaired quality of care. Aim: The aim of this study was to describe nurses' experience of occupational stress and factors that contribute to occupational stress. Method: The method used was a literature review. Results: The analysis resulted in five themes which describe nurses' experiences of occupational stress and factors that contribute to occupational stress. These themes are; The nursing profession's change, Experience and education, High workload, Organization and work environment and Practical nursing. The result in this study shows that the strongest factors contributing to occupational stress in the nursing profession was; administrative work, objectification of patients, nurses' work experience and level of education, understaffing, high workload, time pressure, lack of support from management, cost savings, patient safety and palliative care. The nurses' experienced that their profession is changing and that the management does not appreciate or listen to them. Nurses' experienced feelings of inadequacy and powerlessness when they could not give patients good care because of occupational stress. They were afraid to hurt patients because of the stressful work environment. Conclusion: This study gives a deeper understanding of how nurses experience occupational stress and found several factors contributing to occupational stress. These findings can be used by health management and others to prevent occupational stress for nurses.

    Background: We are facing a global lack of working nurses since nurses end their carrier earlier than their planned retirement. The psychosocial work environment is a big problem in the nursing field and affects the nurses´ health status, suffering and stress level.

    Aim: The aim with this study was to describe nurses´ perceptions of factors that affects their psychosocial working environment.

    Method: A literature overview

    Results: The collegial relationships, the workplace manager, the structure of the working hours and the organization and staffing makes the ground for however the nurses will appreciate and stay at their workplace, or if they are going to dislike the work and leave either the workplace or the nursing-field entirely.

    Conclusion: Adequate staffing, good relations with the co-workers, a manager that sees and appreciates the nurses is work environmental factors that increase the chances that the nurses is going to appreciate their work, and remain at their workplace. To achieve a good work environment there should be enough time for the nurses´ to take good care of the patients and colleagues. There should also be a supporting management at the work place and a good cooperation in the working team. A mean manager, understaffing, bad relationships with colleagues, and a lack of trust and independence increases the risk that the nurses will leave both the workplace and the entire nursing field.

    Epilepsy can sometimes be followed by memory impairment. This can result from the underlying cause of epilepsy or from recurrent seizures, or can be a side effect of antiepileptic drugs or a symptom of another disease such as depression. The aim of the study described here was to explore the experience of living with epilepsy and subjective cognitive decline.

    Method

    To better understand the deeper meaning of the phenomenon, a qualitative design was chosen. Fourteen adults aged 18–35 took part in focus group interviews. The participants were divided into four groups, two groups of women and two groups of men, and the interviews were conducted according to a semistructured protocol. Transcripts were analyzed in accordance with the content analysis guidelines.

    Results

    Four themes emerged: “affecting the whole person,” “influencing daily life,” “affecting relationships,” and “meeting ignorance in society.”

    Conclusions

    Cognitive decline has a heavy impact on young adults with intractable epilepsy. In contrast to seizures, the cognitive decline is persistent. The themes reflected different hardships faced by the participants. The consequences of living with epilepsy and cognitive impairment concerned education, employment, social life, self-esteem, and hope for the future. The participants were already using strategies to cope with their cognitive decline, but may benefit from help in developing new strategies to better adjust to their memory problems. Development of more educational programs for both people with epilepsy and their relatives could improve their difficult situations. With help, people can learn to adjust their goals in life and live a fulfilling life despite the disease.

    The standard of reporting in diagnostic studies has generally been low. Fortunately, this issue has begun to be addressed in recent years through the discussion of important methodological issues in educational series, textbooks, and checklists. Double-blind, placebocontrolled, oral food challenges (DBPCFC) are considered to be the gold standard for diagnosis of food allergy. However, there is no consensus regarding how to interpret the outcome and how to defi ne positive and negative provocations in DBPCFC. Furthermore, since most theories on the diagnosis of food allergy rely on the assumption that the DBPCFC has a high accuracy, this accuracy must be formally statistically evaluated. In this review, we discuss essential methodological issues for diagnostic accuracy studies in general and for oral food challenges in particular and discuss the importance of methodological issues as a guide for forthcoming studies of diagnostic procedures.

    BACKGROUND: Subjective symptoms after food challenges are difficult to interpret and no standard is available. We discuss a strategy for how to interpret a diary. Furthermore, the interobserver reliability is evaluated. METHODS: Diaries for 32 patients with subjective symptoms were used. The diaries were re-evaluated with a predefined strategy by three independent observers. RESULTS: The proportion of positives was 21.9% among the old diagnoses, according to the new approach 34.4% (observers I and II) and 37.5% (observer III) were positive. The new approach had high interobserver reliability (97 and 100%). CONCLUSIONS: The proportion of positives depends on how subjective symptoms are interpreted. Interpretations of subjective symptoms in diaries could be made with high interobserver reliability.

    Abstract [en]

    Introduction. An important factor for delayed healing of chronic wounds is the presence of bacteria. Quorum sensing (QS), a cell density-dependent signalling system, controls the production of many virulence factors and biofilm formation in Pseudomonas aeruginosa.Aim. Inhibition by sodium salicylate (NaSa) of QS-regulated virulence expression was evaluated in QS-characterized clinical wound isolates of P. aeruginosa, cultured in serum-containing medium.Methodology. Fourteen clinical P. aeruginosa strains from chronic wounds were evaluated for the production of QS signals and virulence factors. Inhibition of QS by NaSa in P. aeruginosa clinical strains, wild-type PAO1 and QS reporter strains was evaluated using in vitro assays for the production of biofilm, pyocyanin, siderophores, alkaline protease, elastase and stapholytic protease.Results. Six clinical strains secreted several QS-associated virulence factors and signal molecules and two were negative for all factors. Sub-inhibitory concentrations of NaSa downregulated the expression of the QS-related genes lasB, rhlA and pqsA and reduced the secretion of several virulence factors in PAO1 and clinical strains cultured in serum. Compared to serum-free media, the presence of serum increased the expression of QS genes and production of siderophores and pyocyanin but decreased biofilm formation.Conclusions.Pseudomonas aeruginosa from chronic wound infections showed different virulence properties. While very few strains showed no QS activity, approximately half were highly virulent and produced QS signals, suggesting that the targeting of QS is a viable and relevant strategy for infection control. NaSa showed activity as a QS-inhibitor by lowering the virulence phenotypes and QS signals at both transcriptional and extracellular levels.

    Background:

    Foster care can be an alternative if a child cannot stay in their biological family. Many children in foster care do not have a good mental health because of their situation and earlier experiences. John Bowlby's attachment theory provides an explanation of mental health and development in children. It is important for children to have at least one person to attach to. Aim: The aim of this study was to investigate factors that affect the mental health and development of children in foster homes. Method: In this literature review nine qualitative and quantitative articles were analyzed, which resulted in three themes. Results: The first theme deals with how important the foster parents are. How they act and how they support their foster children affect the children's mental health. The second theme describes the importance of stability in the foster home and the last theme describes the effect that continuity of school and other activities have on the mental health and development of the foster children. Conclusion: Children in foster care need homes where they can attach to their foster parents. It is important for the children to have and feel stability in one foster home and that they are not forced to move to new foster homes. Continuity in school and activities has an important role in making them involved in family and it is critical that they can keep contact with friends and other people who are involved in their activities

    Abstract [en]

    Structural and chemical properties of the small water clusters W-4, W-5 and W-6 are investigated with the theory of atoms and molecules (QTAIM). For the W-4, W-5 and W-6 clusters, nine, fourteen and twenty-seven conformers, respectively, have been analyzed. For the W-4, W-5 and W-6 clusters one, two and three of these structures, respectively, have not been reported before. We then proceed to extend the W-4, W-5 and W-6 water cluster topology space using QTAIM; the Poincare-Hopf topological sum rules are applied to create rules to identify the spanning set of conformer topologies, this includes finding three, ten and eight new distinct topologies that satisfy the Poincare-Hopf relation for W-4, W-5 and W-6 respectively. The topological stability of degenerate solutions to the Poincare-Hopf relation is compared by evaluating the proximity to rupturing of critical points of the gradient vector field of the charge density. We introduce a QTAIM topology space to replace the inconsistent use of Euclidean geometry to determine whether a cluster is 1-, 2- or 3-D. We show from the topology of the charge density that the conformers of the W-4, W-5 clusters are more energetically stable in less compact, planar forms, conversely the conformers of W-6 are more energetically stable with compact 3-D topologies. Quantifying the degree of covalent character in the hydrogen bonding for the W-4, W-5 and W-6 clusters independently verifies this finding. Differences in simple rules for the number of hydrogen bonds obeying the Bernal-Fowler ice rules between W-4, W-5 and W-6 reflect the transition from 2-D to 3-D structures being more energetically stable. In addition, we identify a new class of O-O bonding interactions that are up to 48% longer than the inter-nuclear separation and appear to be failed hydrogen bonds.

    Abstract [en]

    OBJECTIVESFatigue after an acquired brain injury is common, and is characterized by limited energy reserves to accomplish ordinary daily activities. A typical characteristic of mental fatigue is that the mental exhaustion becomes pronounced during sensory stimulation or when cognitive tasks are performed for extended periods without breaks. There is a drain of mental energy upon mental activity in situations in which there is an invasion of the senses with an overload of impressions, and in noisy and hectic environments. Another typical feature is a disproportionally long recovery time needed to restore the mental energy levels after being mentally exhausted. The mental fatigue is also dependent on the total activity level as well as the nature of the demands of daily activities. For many people, there is an increased risk of doing too much and becoming even more fatigued.METHODSWe have developed an application for Windows Phone for assessment of mental fatigue. The Mental Fatigue Scale is used. The MFS is a multidimensional questionnaire containing 15 questions. The questions included in the MFS are based on symptoms described following longitudinal studies of patients with TBI, brain tumours, infections or inflammations in the nervous system, vascular brain diseases, and other brain disorders. The app also includes information about mental fatigue.   RESULTSThis application can help people determine the level of mental fatigue and it can also serve to provide an overall picture of the severity of the condition, and detect changes in mental fatigue over time. The scores will be added up and the results will be presented in the form of a rating scale and a diagram. People can then see their results for one week ago, one month ago or a whole year ago. Today, the most important recommendations are to adapt to the energy available by doing one thing at a time, resting regularly and not overdoing things. However, this is challenging for most people and it may take a long time, even years, to adapt to a sustainable level. It may also be difficult for the person to learn by himself/herself and it can take several years of considerable struggle, frustration, despair and depression, to find the right balance between rest and activity. This app can help people to be aware of mental fatigue. If they connect the results to daily activities, the app may also help them to be more aware about what may alleviate and what may make mental fatigue worse. CONCLUSIONSWith regular assessment of mental fatigue, this app may give feedback and support in order to achieve an enduring balance between activities and rest.

    Background: Person-centered care sciences are experiencing rapid progress. Personalization in care services is becoming the norm, and implementation from scientific knowledge is increasingly acknowledged and mandated. Advances in personality and brain disorder research are crucial in assisting the future development of personalized care. Aim: We will attempt to present glimpses into the future of personalized care with support from frontline science, measurement, and practice, updating with input from personality genetics and measurement theory. Outline: We present three broad developments: (1) scientific advancements in understanding how personality and genetics are central in predicting mental health and disorders, with the potential to increase predictive diagnosis and treatment validity; (2) measurement advancements with help of trait dimensions and latent structures, with the potential to increase reliability in assessing personalized care needs and functioning; (3) practical advancements in implementing a personalized approach in care services, with the potential to increase effectiveness and satisfaction with patients. We review this glimpse into the future by referencing key findings in personality and assessment meta-analyses, genome-wide association studies (GWAS), and trait measurements in psychiatric disorders. Conclusion: Personalizing care services will benefit practitioners and patients. We suggest and recommend that personalized care diagnosis and treatment is the way forward and that the future will be potentially revolutionized by incorporating the presented advancements in personality research and brain sciences.

    The background for the present study is that facial width-to-height ratio (fWHR) is suggested to function as an evolutionary cue for threatening behavior such as perceived aggression. With a novel approach, in this pilot study, we explored whether fWHR can be detected in observers’ brain responses measured by event-related potentials (ERP), specifically, the Late Positive Potential (LPP) component (400-3000ms after stimuli onset). The hypothesis was that faces with a high fWHR would elicit a larger LPP amplitude than faces with a low fWHR. The results showed that faces with high fWHRs were indeed perceived as more aggressive and elicited significantly greater LPP amplitudes. The conclusion lends initial support to fWHR serving as a facial cue with evolutionary relevance. We caution that future full-length studies need to take the current small-scale study’s limitations into consideration.

    Background: During 2014 a total of 89062 people died in Sweden. Nurses' will meet patients who are in the end of their life. The work with dying patients can be emotionally draining but also very rewarding for the nurses.

    Aim: The aim of this study was to explore nurses' experiences of meeting the dying patients and their close ones.

    Method: The method used is a litterature based study with focus on qualitative studies. The articles were processed using the Friberg method of analysis.

    Results: There were ten articles which were analysed and compiled to a result. There are three themes and ten subthemes. The first theme is about seeing the work as a privilege - it's about happiness, gratefulness and benevolence. The second theme is about feeling frustration - it's about not being ready to meet the patient and their close ones, being uncertain and being alone during work. The third and last theme is to experience stress - it's about feeling pressure, feel sadness and to feel stress and anxiety.

    Conclusion: Both positive and negative experiences are shown in the result. The palliative care isn't just about caring for the patient, but also about supporting their next of kin in their mourning process. The nurses experiences can sometimes reflect in their work towards the patient and their kin, which puts the nurses in a difficult situation.

    Background:

    Sleep is crucial to maintaining good mental and physical health, it helps to repair and restore our brain and body. Sleep affects our language abilities, ability to concentrate, reading comprehension and the brain's ability to summarize what we hear. If our sleep is neglected, it compromises our performance, our mood and our interpersonal relationships. Sleep deprivation can lead to impaired decision-making capacity and increased risk-taking. This may cause the work performance to deteriorate.

    Aim:The aim of this study was to investigate sleep deprivation by nurses and its impact on work capacity.

    Method:A literature review based on an analysis of quantitative research.

    Results:The result showed that sleep deprivation often resulted in the nurses’ having difficulty staying awake during the daytime, and involuntary sleep episodes during working hours occurred. Lack of sleep had a negative effect on nurses' job performance, which often resulted in accidents and medical injuries related to decreased concentration and attention. Sleep deprivation was associated with the presence of depression and anxiety. Sleep deprivation also resulted in increased absenteeism, more health care visits, and somatic symptoms.

    Conclusion/Clinical significance: Nurse’s sleep should be fulfilled, as it is an essential basic need. Sleep deprivation can affect the nurse negatively and consequently patient safety. Since sleep deprivation can be equated with intoxication, the authors consider that it is crucial to recognize and discourage sleep deprivation to prevent healthcare-associated injuries

    Background:

    The aging population in the world has increased and many people are reaching very high ages. This has made palliative care important because the aging population requires more palliative care. India is classified as a developing country where the aging population over the past 20 years has increased. How Indian nurses maintain patients’ autonomy in palliative care is important to study to gain more knowledge and understanding among nurses.

    Aim:

    The aim of this study was to illuminate Indian nurses’ experiences of maintaining the elderly patients’ autonomy in palliative nursing care.

    Method:

    A qualitative content analysis was conducted in order to achieve variations of nurses’ experiences of maintaining patients’ autonomy in palliative nursing care. Six interviews were conducted. The interviews took place in a private hospital in east India.

    Results:

    The result shows one theme and two categories of value for nurses to maintain the patients’ autonomy in palliative care. The theme is: nurses’ compassion and respect for the patient. The categories are: to make the patients involved in their own care and to communicate with the patients.

    Conclusion:

    When supporting patients in palliative care, nurses need knowledge about these mentioned areas, so they can find strategies to meet the patients with respect and to maintain their autonomy

    Background: Every year half a million women get diagnosed with cervical cancer. Cervical cancer affects women in several aspects of life and is the cause of many different experiences, both physical and psychological.

    Aim: The aim of this study was to illuminate women with cervical cancer and their experiences in daily life.

    Method: A literature review has been made of study five qualitative and five quantitative articles. Articles were analyzed by searching for similarities and differences. The studies were published between 2000 and 2013.

    Results: The results of the study appeared in three categories: The physical experience of cervical cancer, the psychological experience of cervical cancer and the social experiences of cervical cancer. The result showed that diarrhea, worry and anxiety were the most common problems experienced by women living with cervical cancer.

    Conclusion: The results of this study showed that the physical side effects of cervical cancer had an essential consequence on the psychological and social life of the affected women

    Fibromyalgia is a chronic pain disorder. It affects muscles and connective tissue. It's a very complex disorder that has no adequate treatment or cure. The research has progressed but there are still some questions to be answered. Quality of life for these patients is decreased and leads to suffering. Aim: The aim of this study was to illuminate patients' experiences of living with fibromyalgia. Method: A literature overview where nine different qualitative studies between the years 1998-2012 were analyzed. Results: The analysis of the articles lead to six subthemes "Accepted and adapted to the new life", "Experienced a new I", "Experienced changes in the work situation", Experienced changes in privacy", Experienced ignorance" and "Experienced incomprehension" which lead to three main themes "Experienced an identity change", "Experienced a sense of isolation" and "Felt frustration in meeting with health care" Conclusion: The disorder affects the patients both mentally and physically. More research needs to be done for more knowledge about the disorder in order to improve patients care and quality of life

    Background: Nurses often encounter traumatic events at work, but how do they handle and process the feelings that comes with that? And do they have the time to cope with the feelings afterwards?

    Aim: To highlight nurses' experiences of dealing with traumatic care situations.

    Method: A literature-based study with the analysis of qualitative research. Results: Three themes emerged from the results. Sense of belonging, keeping a distance and experience provides development. Sense of belonging shows that the feeling of being part of a team and being able to share emotions helps the nurses to deal and cope with traumatic situations. Keeping a distance shows that the nurses sometimes feel the need to keep their feelings on a distance to be able to deal with traumatic situations. Experience provides development shows that time and experience gives the nurses confidence and resistance which make it easier for them to deal with traumatic situations.

    Conclusion: The team makes a big difference in how the nurse will be able to deal with and process the feelings after encountering a traumatic situation at work. The number of traumatic experiences also played a role in how the nurses were able to deal with and process their feelings. With the experiences came the resistance which was good in the process of dealing with traumatic situations.

    This study compared professional and amateur classical musicians matched for age, gender, and education on reaction times during the Stroop color-word test, brainwaves during an auditory ERP task and during paired reaction-time tasks, responses on the Gibbs Sociomoral Reflection questionnaire, and self-reported frequencies of peak experiences. Professional musicians were characterized by: (1) lower color-word interference effects (Stroop task), (2) faster categorization of rare expected stimuli (P3b), and a trend for faster processing of rare unexpected stimuli (P3a), (3) higher scores on the Sociomoral Reflection questionnaire, and (4) more frequent peak experiences during rest, tasks, and sleep. Both groups had high values on the Brain Integration Scale. These findings are interpreted in light of a Unified Theory of Performance, which posits that effectiveness in any area is influenced by one’s level of mind-brain development—emotional, cognitive, moral, ego and cortical development—with higher mind-brain development supporting greater effectiveness in any domain.

    Work-related stress is relatively common in modern society and is a major cause of sick-leave. Thus, effective stress reducing interventions are needed. This study examined the effects of mental training and mechanical massage, on employee's heart rate variability (HRV) and plasma cortisol at their workplaces. Moreover, it was investigated whether baseline systolic blood pressure (SBP) can explain differences in effectiveness of the intervention. Ninety-three participants from four workplaces were randomly assigned to one of the five programs: (I) Mechanical massage and mental training combined, II) Mechanical massage, III) Mental training, IV) Pause, or V) Control. HRV and plasma cortisol were measured at baseline and after 4 and 8 weeks. SBP was measured at baseline. On the reduction of cortisol levels, a small effect of the mechanical massage program was found, whereas no effect was found for the other programs. None of the programs showed any effect on HRV. Nonetheless, when the level of systolic blood pressure was taken into account, some small beneficial effects on HRV and cortisol of mental training and the mechanical massage were found. This exploratory pilot-study provides useful information for future studies that aim to reduce stress among employees.

    Abstract [en]

    PURPOSE: Most of the patients who receive myeloablative therapy prior to stem cell transplantation develop oral mucositis (OM). This adverse reaction manifests as oral mucosal erythema and ulcerations and may require high doses of morphine for pain alleviation. OM may also interfere with food intake and result in weight loss, a need for parenteral nutrition, and impaired quality of life. To date, there have been very few studies of evidence-based interventions for the prevention of OM. Cryotherapy, using ice chips, has been shown to reduce in an efficient manner the severity and extent of OM, although clinical applications are still limited due to several shortcomings, such as adverse tooth sensations, problems with infectious organisms in the water, nausea, and uneven cooling of the oral mucosa. The present proof-of-concept study was conducted to compare the tolerability, temperature reduction, and cooling distribution profiles of an intra-oral cooling device and ice chips in healthy volunteers who did not receive myeloablative treatment, and therefore, did not experience the symptoms of OM.

    METHODS: Twenty healthy volunteers used the cooling device and ice chips for a maximum of 60 min each, using a cross-over design. The baseline and final temperatures were measured at eight intra-oral locations using an infra-red thermographic camera. The thermographic images were analysed using two digital software packages. A questionnaire was used to assess the tolerability levels of the two interventions.

    RESULTS: The intra-oral cooling device was significantly better tolerated than the ice-chips (p = 0.0118). The two interventions were equally effective regarding temperature reduction and cooling distribution.

    CONCLUSIONS: The intra-oral cooling device shows superior tolerability in healthy volunteers. Furthermore, this study shows that temperature reduction and cooling distribution are achieved equally well using either method.
