Bakgrund

Hjärtsvikt är en globalt utbredd diagnos med hög dödlighet. Personen med hjärtsvikt lever med många fysiska symtom som påverkar det dagliga livet. Cirka 10 procent av Sveriges befolkning vårdar någon de står nära på grund av sjukdom. Allt fler personer väljer att stanna kvar i hemmet även mot livets slut. Vid måttlig till svår hjärtsvikt är det svårt för personer med hjärtsvikt att klara sig själva och de behöver då stöd av anhöriga med vardagssysslor och med den personliga vården.

Syfte var att beskriva anhörigas upplevelser när personen med hjärtsvikt vårdas i hemmet.

Metoden som användes var en systematisk litteraturöversikt. Sjutton artiklar inkluderades i studien och dessa analyserades med innehållsanalys.

Resultatet

Mynnade ut i fyra kategorier med underkategorier. Första kategorin: Emotionellt påverkan handlar om oro och maktlösheten som anhöriga känner i relation till diagnosen, om hur det sociala stödet spelar in och även behovet av en stödjande familj. Den andra kategorin: Relationen förändras handlar om kärlek plikt och självuppoffring, behov av ömsesidig omvårdnad samt upplevelsen av meningsfullhet i att vård. Den tredje kategorin: En förändrad vardag, innehåller det ökade ansvaret som landar på den anhöriga, hur den anhörige anpassar dagen efter personen med hjärtsvikt och de ekonomiska utmaningar som kan drabba anhöriga. Fjärde kategorin: Informationsbehov och stöd från vården innehåller önskan om bra kommunikation, behov av kontinuerlig information och stöd från vårdpersonalen samt hur anhöriga upplever sig som ofrivilliga samordnare av personen med hjärtsvikts vård.

Slutsatsen var att stöd och information är två viktiga aspekter för anhöriga att fortsätta orka vårda. Kontinuitet och en direktkontakt med sjukvården som vårdcentral eller ett specialiserat team är betydelsefullt. Distriktssköterskans roll i att stödja anhöriga för att fortsätta vårda påvisas vara av stor betydelse.

Background

Heart failure is a globally widespread diagnosis with high mortality. The person with heart failure lives with many physical symptoms that affect daily life. About 10 percent of Sweden's population cares for someone they are close to due to illness. More and more people choose to remain at home, even towards the end of life. In the case of moderate to severe heart failure, it is difficult for people with heart failure to cope on their own and they then need support from relatives with everyday tasks and with personal care.

The aim was to describe the experiences of relatives when the person with heart failure is cared for at home.

The method used was a systematic literature review. Seventeen articles were included in the study and these were analyzed using content analysis.

The result resulted in four categories with subcategories. First category: Emotional impact is about anxiety and the powerlessness that relatives feel about the diagnosis, about how social support comes into play and the need for a supportive family. The second category: The relationship changes is about love, duty and self-sacrifice, the need for mutual care and the experience of meaningfulness in care. The third category: A changed everyday life, includes the increased responsibility that lands on the relative, how the relative adapts the day to the person with heart failure and the financial challenges that can affect relatives. The fourth category: Information needs and support from care includes the desire for good communication, the need for continuous information and support from the care staff, and how relatives perceive themselves as involuntary coordinators of the person with heart failure's care.

The conclusion was that support and information are two important aspects for relatives to continue caring. Continuity and direct contact with healthcare such as a healthcare center or a specialized team is important. The district nurse's role in supporting relatives to continue care is shown to be of great importance.
