Bakgrund:

Palliativ vård i hemmet blir allt vanligare och är en stor del av distriktssköterskans arbete i hemsjukvården. Arbetet kring den palliativa patienten är komplext och ställer höga krav på distriktssköterskans kompetens, kunskap och erfarenhet. Distriktssköterskan ska ha kunskap inom många områden och kunna samarbeta med både patienter, anhöriga och andra professioner för att kunna bedriva god palliativ vård. Dessa höga krav kan leda till känslor av otillräcklighet, utsatthet och ensamhet i rollen som distriktssköterska i hemsjukvården.

Syfte:

Att belysa distriktssköterskors upplevelser av att utföra palliativ vård i hemmet.

Metod:

En kvalitativ systematisk litteraturöversikt utifrån Evans metod. Litteratursökning gjordes i databaserna Cinahl och PubMed. 13 artiklar godkändes och låg till grund för resultatet.

Resultat:

Resultatet redovisades i tre teman som var: Det stora ansvaret, Samarbetets betydelse för palliativ vård och Att vara människa. I resultatet framkom det att distriktssköterskorna har både positiva och negativa upplevelser av att utföra palliativ vård i hemmet. Några viktiga faktorer som påverkade distriktssköterskornas upplevelse var samarbetet med läkare och sjuksköterskekollegor, samarbetet med patienter och anhöriga, arbetsbördan samt vilket stöd de fick både emotionellt och professionellt.

Slutsats:

Arbetet med att utföra palliativ vård i hemmet är utmanande för distriktssköterskorna och ställer höga krav på deras kompetens och erfarenhet. Distriktssköterskan fungerar som spindeln i nätet i samband med den palliativa vården och det är därför viktigt att distriktssköterskan får tillgång till de resurser och det stöd som krävs för att bedriva god palliativ vård i hemmet.

Background:

Palliative home care is becoming more common and is a big part of the district nurse’s work in home health care. The work with the palliative patient is complex and places high demands on the district nurse’s competence, experience and knowledge. The district nurse needs to be able to collaborate and communicate with both patients, relatives and other professions to be able to provide good palliative care. These high demands can often lead to feelings of inadequacy, vulnerability and loneliness when working as a district nurse in home health care.

Aim:

To illuminate district nurses' experiences of providing palliative care at home.

Method:

A qualitative systematic literature review based on Evans' method. A literature search was made in the databases Cinahl and PubMed. 13 articles were approved and formed the basis of the result.

Results:

The results were presented in three themes which were: The great responsibility, The importance of collaboration for palliative care and Being human. The results showed that the district nurses have both positive and negative experiences of providing palliative care at home. Some important factors that influenced the district nurses' experience were the collaboration with doctors and nursing colleagues, the collaboration with patients and relatives, the workload and the support they received both emotionally and professionally.

Conclusion:

The work of providing palliative care at home is challenging for the district nurses and places high demands on their competence and experience. The district nurse functions as a leader and coordinator when working with palliative patients, and it is therefore important that the district nurse has access to the resources and support required to provide good palliative care at home.
