Bakgrund: I Sverige har psykiska besvär hos barn och ungdomar ökat de senaste decennierna och ett utvidgat hälsofrämjande arbete efterfrågas. Skolsköterskors kompetens är betydande när det kommer till att främja barns och ungdomars psykiska hälsa och skolsköterskor kan genom sin roll verka för alla elevers likaberättigande till en positiv psykisk hälsa.

Syfte: Att belysa skolsköterskors arbete för att främja psykisk hälsa hos barn och ungdomar.

Metod: Totalt 12 vetenskapliga studier har syntetiserats genom ett metaetnografiskt tillvägagångssätt baserat på Noblit och Hares metod för syntetisering av kvalitativa studier.

Resultat: Genom syntetisering av de vetenskapliga artiklarna framträdde två övergripande teman; att låta elevernas behov visa vägen samt att arbeta utifrån skilda perspektiv. De två temana konstitueras av fem subteman; att öka barns och ungdomars känsla av delaktighet, att kunna lyssna och se individen, att skapa positiva relationer med barn, ungdomar och familjer, känsla av ensamhet och utanförskap för skolsköterskor inom skolans interprofessionella samarbete och att uppleva maktlöshet i främjande arbete för psykisk hälsa på en generell nivå.Syntesens resultat visade på vikten av att synliggöra och låta elevers behov visa vägen i det hälsofrämjande arbetet med psykisk hälsa, liksom att strukturer inom organisationen kunde försvåra skolsköterskors arbete med att främja barns och ungdomars psykiska hälsa.

Slutsats: På mikronivå framträdde vikten av att skolsköterskor tydliggör elevers behov, och låter dessa vara vägledande i det hälsofrämjande arbetet med psykisk hälsa. På makronivå framkom att strukturer inom skola och samhälle kunde försvåra skolsköterskors arbete med att främja barns och ungdomars psykiska hälsa och strukturerna kunde för skolsköterskor även skapa känslor av ensamhet och utanförskap. Resultatet visade att det krävs tydligare riktlinjer kring hälsofrämjande arbete, mindre elevgrupper för enskilda skolsköterskor samt ett välfungerande interprofessionellt samarbete. På den organisatoriska nivån framträdde sammantaget att det hos skolsköterskor fanns känslor av ensamhet och utanförskap i det interprofessionella samarbetet, liksom en känsla av maktlöshet i det generella arbetet med att främja psykisk hälsa hos barn och ungdomar. 

Background: During the last decades, mental health problems have increased among children and adolescents in Sweden and there is need for extended work with health promotive activities. The competence of the school nurse is vital when it comes to the promotion of children's and adolescent’s mental health and through their role, school nurses can support the equal rights of all pupils for a positive mental health.

Aim: To illuminate school nurses´ work to promote mental health among children and adolescents.

Method: A total of 12 scientific studies were synthesized through a meta-ethnographic approach based on Noblit and Hares´ methodology for synthesization of qualitative studies.

Results: Through the synthesizing of the scientific articles, two overarching themes emerged; to let the students' needs lead the way, and to work from different perspectives. The two themes are constituted by five sub-themes: to increase children's and young people's sense of participation; to be able to listen and see the individual; to create positive relationships with children, adolescents and families; a sense of loneliness and exclusion for school nurses within the school's interprofessional collaboration; to experience powerlessness in the promotion of mental health at a general level. The results of the synthesis showed the importance of illuminating and allowing students' needs to lead the way in the work with mental health promotion and showing that structures within the organization could complicate school nurses' work to promote children's and adolescents' mental health.

Conclusion: At a micro level, the importance of school nurses clarifying students' needs - and allowing these needs to guide the health promotion work with mental health - emerged. At a macro level, the results of the synthesis showed that structures within schools and society could complicate school nurses' work to promote the mental health of children and young people. Those structures could also create feelings of loneliness and exclusion for school nurses. The results showed that clearer guidelines are needed regarding health promotion work, smaller student groups for individual school nurses is important, as well as well functioning interprofessional collaboration. Overall, at the organizational level, it emerged that school nurses had feelings of loneliness and alienation in interprofessional collaboration, as well as a sense of powerlessness in the general work to promote mental health in children and adolescents.
