Bakgrund: På grund av de förändringar som sker i takt med åldrandet, riskerar äldre människor att i högre utsträckning drabbas av svårläkta sår. Multisjuklighet såsom fetma, diabetes och hjärtoch kärlsjukdom ökar även risken att utveckla svårläkta sår. Svårläkta sår kan bidra till ett stort lidande för patienterna och ökar belastningen på sjukvården. Begreppet svårläkta sår innefattar oftast bensår, trycksår och fotsår som inte läker på sex veckor. Specialistsjuksköterskor inom hemsjukvård lägger mycket tid på att behandla patienter med svårläkta sår. De har ett ansvar i att organisera och leda arbetet kring sårbehandling och att samarbeta med andra yrkeskategorier.Kontinuitet, compliance och personcentrerad vård har visat sig vara begrepp som skapar förutsättningar för en framgångsrik sårbehandling. 

Syfte: Att beskriva specialistsjuksköterskors erfarenheter av att arbeta med sårvård hos patienter med svårläkta sår i hemsjukvård.

Metod: Datainsamlingen är baserad på sex semistrukturerade intervjuer med en specialistsjuksköterska och fem distriktssköterskor som arbetar inom hemsjukvård. Insamlad data har analyserats utifrån en kvalitativ innehållsanalys.

Resultat: Specialistsjuksköterskorna som deltog i studien påtalade de känslor som uppkom i det komplexa arbetet vid behandling med svårläkta sår. Kunskap och kompetens lyfts fram som viktiga faktorer och som bidrar till trygghet. Fortbildning är önskat men erbjuds sällan. God samverkan med kollegor anses betydelsefullt. Bristande stöd från behandlande läkare bidrar till en känsla av otrygghet. Kontinuitet under hela sårbehandlingsprocessen framstår som det mest väsentliga, samtidigt som vårdverksamhetens möjlighet att tillgodose den varierar, vilket kan bidra till många ambivalenta känslor.

Slutsats: Studien kan användas som stöd för att göra vårdverksamheter medvetna om betydelsen av att stötta specialistsjuksköterskor i sitt arbete med patienter som har svårläkta sår och bidra till en tryggare arbetsmiljö. Detta skulle inte bara gagna specialistsjuksköterskorna utan i ännu högre grad de patienter som får vård och behandling.

Background: Due to the changes that occur with age, older people are at greater risk of wounds that are hard to heal. Multiple diseases such as obesity, diabetes and cardiovascular disease also increase the risk of developing hard to heal wounds. Hard to heal wounds can contribute to great suffering for patients and increase the burden on healthcare. The term hard to heal wounds usually includes leg ulcers, pressure ulcers and foot ulcers that do not heal in six weeks. Specialist nurses in home health care spend a lot of time treating patients with hard to heal wounds. They have a responsibility to organize and lead the work on wound care and to collaborate with other professional categories. Continuity, compliance and person-centered care have proven to be concepts that create the conditions for effective wound treatment.

Aim: To describe specialist nurses' experiences of caring and treating patients with hard to heal wounds in home care.

Method: The data collection is based on six semi-structured interviews with one specialist nurses and five district nurses who work in the home care services. A qualitative content analysis was used to analyze the data. Results: The specialist nurses who participated in the study pointed out the positive and negative emotions that arose in the complex work of treating hard to heal wounds. Knowledge is highlighted as very important for the work of district nurses in home health care. Continuous education is desired but is rarely offered within the organization. Good collaboration with other professions and continuity are also highlighted. Lack of support from treating doctors affects the feeling of security. The business opportunity for continuity varies and could contribute to many ambivalent emotions.

Conclusion: The study may be used as support to make organizations aware of the importance of supporting district nurses in their work of treating patients with hard to heal wounds, which would contribute to the feeling of safety at work. This can improve the opportunities to heal wounds and contribute to a better working environment.
