Abstract [sv]

Bakgrund: Primärvården ska verka som navet för att kunna ge patienten en god, nära och samordnad vård. För att kunna arbeta personcentrerat och sätta patienten i centrum behöver samverkan mellan vårdaktörer förbättras. Samverkanslagen kom till för att patienter inte skulle “hamna mellan stolar” vid exempelvis utskrivning från sjukhus. 

Syfte: Syftet var att undersöka hur sjuksköterskor och distriktssköterskor på vårdcentral erfar samverkan med andra vårdaktörer i vården av svårt sjuka patienter som inte kan ta sig till sin vårdcentral.

Metod: En tvärsnittsstudie via digital semistrukturerad enkätundersökning samt en kvalitativ, förtydligande del. Kvalitativa svar analyserades med innehållsanalys.

Resultat: Resultatet visar att så gott som alla sjuksköterskor samarbetar med andra vårdaktörer såsom hemsjukvård, rehab och sjukhusmottagningar. De frågor som besvarades med egna ord kodades till kategorier och teman eller sammanfattades i text. Resultatet av denna visar att sjuksköterskor vill arbeta mer personcentrerat för att göra patienten delaktig. Resursbrist och tidsbrist gör att vården hamnar i fokus istället för patienten.

Slutsats: Vårdpersonal vill samverka men trots flera försök att förbättra samverkan i vården är det fortfarande stora brister. Rutiner och resurser saknas för att få det att fungera.

Abstract [en]

Background: Primary care should act as the hub for providing the patient with good, close and coordinated care. In order to be able to work person-centered and put the patient at the center, collaboration between healthcare actors needs to be improved. The collaboration law was created so that patients would not “slip through the cracks” when, for example, discharged from hospitals.

Purpose: The purpose was to investigate the experiences of nurses and district nurses working at a health center of collaboration with other care actors in the care of seriously ill patients who are unable to go to their health center.

Method: Cross-sectional study via digital semi-structured survey. Qualitative responses were analyzed with content analysis.

Results: The result shows that almost all nurses cooperate with other healthcare providers such as home care, rehab and hospital clinics. The questions answered in their own words were encoded into categories and themes. It is clear that more nurses want to work more person centered to involve the patient. Lack of resources and lack of time means that care is in focus instead of the patient.

Conclusion: Healthcare professionals want to collaborate, but despite several attempts to improve collaboration in healthcare, there are still major shortcomings. Routines and resources are lacking to make it work.
