Abstract [sv]

Bakgrund: Andelen hemsjukvård i Sverige har ökat de senaste åren i takt med att slutenvårdsplatserna minskat i antal. Patienter med komplexa vårdbehov vårdas i allt större utsträckning i hemmet. För att möta detta behov av mer avancerad vård i hemmet har olika modeller tagits fram. I Västra Götalandsregionen infördes närsjukvårdsteam, som ska stötta vården av patienter med komplexa vårdbehov i hemmet. Modellen kräver en välfungerande samverkan mellan kommun, primärvård och slutenvård. Sjuksköterskan i hemsjukvården fungerar fortfarande som patientansvarig sjuksköterska när deras patienter är inskrivna i närsjukvårdsteam. Det saknas forskning om hur sjuksköterskorna i hemsjukvården upplever samverkan med denna nya vårdform. Syfte: Att beskriva sjuksköterskan i hemsjukvårdens upplevelser av att samverka med ett närsjukvårdsteam.

Metod: Semistrukturerade intervjuer med tio sjuksköterskor i hemsjukvård från tre kommuner i Västra Götalandsregionen. Analys genomfördes med kvalitativ innehållsanalys.

Resultat: Tre kategorier och nio underkategorier framkom. Resultatet visar att sjuksköterskorna upplevde stora fördelar med närsjukvårdsteamet, både i sitt arbete och för patienterna. Sjuksköterskorna kände ett stort ansvar för sina patienter, vilket inte förändrades vid samverkan med närsjukvårdsteamet. Sjuksköterskorna upplevde att inte alla patienter som hade behov av det blev inskrivna i närsjukvårdsteamet blev det. Sjuksköterskorna hade en bristande kunskap om hur närsjukvårdsteamet fungerade.

Slutsats: Närsjukvårdsteam har potential att leda till en mer personcentrerad vård i hemsjukvården och ett bättre utnyttjande av vårdens resurser. Kunskapsbristen kring närsjukvårdsteamet riskerar dock i nuläget att leda till att vården för patienterna i hemsjukvården blir ojämlik. Kunskapen om teamet behöver öka och tydligare samverkansrutiner behöver införas.

Abstract [en]

Background: The proportion of home health care in Sweden has increased in recent years, as the number of beds in hospital care have decreased. Patients with complex care needs are being cared for at home in greater capacity. To meet the need for more advanced home care different solutions have been created. In the region of Västra Götaland integrated mobile care teams was introduced, that are meant to support the care of patients with complex care needs in their homes. The model requires a well-functioning cooperation between municipality-based home health care, primary health care and hospital care. The nurse working in municipal health care services is still the primary nurse for the patient when the patient is enrolled in the integrated mobile care team. There is a lack of research of how the nurses working in municipal health care experience this new model of care.

Aim: To describe how the nurse working in municipal health care experiences working with an integrated mobile care team. Method: Semi-structured interviews with 10 nurses working in municipal health care in 3 different municipalities in the region of Västra Götaland. The analysis was conducted with qualitative content analysis.

Results: Three categories and nine sub-categories emerged. The result shows that the nurses experienced advantages with the integrated mobile care team, both in their own work and for their patients. The nurses felt a great responsibility for their patients, which didn’t change with the patients’ enrollment in the integrated mobile care team. The nurses did not experience that all patients that needed the help of the integrated mobile care team got help from the team. The nurses were lacking knowledge of the integrated mobile care team. Conclusion: The integrated mobile care team has the potential to create a more person-centered care in municipal health care, and a better utilization of resources in health care. The lack of knowledge about the integrated mobile care team risks at the moment to lead to a more unequal care for the patients in municipal health care. The knowledge about the integrated mobile care team needs to increase, and more well-defined collaboration routines need to be established.
