Abstract [sv]

Bakgrund: Vikten av hälsofrämjande arbete inom hälso- och sjukvård speglas tydligt i forskning, litteratur, riktlinjer och lagar. Distriktssköterskan förväntas ha ett grundläggande hälsofrämjande fokus. På sikt kan detta leda till en bättre folkhälsa och spara lidande och resurser för samhälle och individ. Ändå verkar fokus ofta ligga på att behandla sjukdom. Enligt tidigare forskning upplever distriktssköterskor som arbetar i primärvård olika hinder trots att de har viljan att arbeta mer hälsofrämjande.

Syfte: Belysa distriktssköterskans erfarenheter av hälsofrämjande arbete i primärvården. 

Metod: Studien genomfördes med en kvalitativ innehållsanalys av material insamlat genom semistrukturerade intervjuer. Sju intervjuer genomfördes i Göteborg med omnejd. Deltagarna skulle ha arbetat minst ett år som distriktssköterska.

Resultat: Analysen resulterade i temat; Det är enklare att gå en upptrampad stig än att gå genom snår för att komma till en glänta framkom. Temat byggde på kategorierna; Det inre drivet, Finns det tid finns det hopp, En spindel i nätet, Sprickan mellan teori och praktik.

Slutsats: Distriktssköterskorna hade en positiv inställning och vilja till hälsofrämjande arbete och ansåg att det innefattade personcentrerad vård. Det hälsofrämjande arbetet beskrevs i termer av små insatser invävda i det dagliga arbetet. Det framkom att de huvudsakliga arbetsuppgifterna är sjukdomsorienterade, vilket huvudsakligen beskrevs som att det saknas resurser för hälsofrämjande insatser.

Abstract [en]

Background: The importance of health promotion within nursing and health care is clearly reflected in science, literature, guidelines and regulations. The district nurse is expected to have an underlying focus on health promotion. In the long run this can lead to a better public health and spare the society and individual suffering and resources. Despite this, it appears that the focus often remains on threating disease. According to previous studies district nurses in the primary care are experiencing obstacles despite having the will and intention to work more towards health enhancement.

Aim: Illuminate district nurses experiences of health promotion within the primary health-care system.

Method: The study has been performed using a qualitative content analysis of material gathered by semi-structured interviews. A total of seven interviews has been performed in Gothenburg and surrounding areas. The participants of the interviews had worked as a district nurse for a period of at least one year.

Results: The analysis resulted in the theme; "It is easier to walk upon a trail than find ones way though the thickets to get to the glade" was noticed. The theme was constructed out of the categories; The inner drive, If there is time there is hope, A spider in the web, The divergence between theory and practice.

'Conclusion: The district nurses in the study had a positive attitude towards health promotion and considered it involved in person-centered care. The health promoting work was described in terms of small efforts woven into the daily work. It emerged that the main duties is focused on disease treatment, which mainly related to resources lacking for health promoting efforts.
