Abstract [sv]

Bakgrund: Vården kring äldre personer är komplex då många äldre är multisjuka vilket också medför att ett stort antal olika vårdgivare behöver samspela. Flexibilitet och ömsesidigt förtroende samt tillit krävs för att samarbetet ska fungera. Äldre personer har rätt till delaktighet och god palliativ vård vid livets slut oavsett var de befinner sig. Sjuksköterskan innehar en betydande funktion i att främja den äldre patientens delaktighet i dennes vård vid livets slut.

Syfte: Syftet med studien var att beskriva hur sjuksköterskan främjar den sköra äldre patientens delaktighet i vårdandet vid livets slutskede på geriatrisk akutvårdsvårdavdelning.

Metod: Datainsamlingen bestod av tre semistrukturerade fokusgruppintervjuer där totalt tio sjuksköterskor deltog. Analysen genomfördes med kvalitativ innehållsanalys.

Resultat: I resultatet framkom tre övergripande kategorier och till dessa åtta subkategorier. De kategorier som framkom var; Samverkan, Lyhördhet för patientens vilja och Omgivande miljöfaktorer.

Konklusion: För att främja den äldre patientens delaktighet i sin vård vid livets slut är det betydelsefullt att patientens vilja avspeglas i vårdandet. Det är centralt att alla inblandade parter har ett gemensamt synsätt på vården för att kunna tillgodose den äldres önskemål.

Abstract [en]

Background: The care of older people is complex and are affected by several medical conditions and involve a large number of different caregivers that need to cooperate. Flexibility and mutual trust are required for the collaboration to work. Older people have the right to participate and receive good palliative care at the end of life regardless of where they are and which caregivers is responsible. The nurse has a significant role to play in promoting the older patient's participation in his or her end of life care.

Aim: The aim of the study was to describe how nurses facilitate participation for frail older people in end of life care in geriatric emergency wards.

Method: The data collection consisted of three semi structured focus group interviews where a total of ten nurses participated. The analysis was conducted with qualitative content analysis.

Results: The results revealed three main categories and eight subcategories. The categories that emerged were; Collaboration, Responsiveness to the patient's will and Surrounding environmental factors.

Conclusion: To promote the older patient's participation in care at the end of life, it is important that the patient's desires is reflected in the care. It is crucial that all parties involved have a common vision to meet the wishes of the older people.
