Implementering av IT inom hälso- och sjukvården anses av många både kunna förbättra vården, genom att bland annat göra den mer effektiv, och ge ekonomiska fördelar, till exempel genom att vårdgivare kan spara pengar. Samtidigt finns det studier som visar på att det kan uppstå problem vid införandet av nya teknologier inom hälso- och sjukvården. Problem som har rapporterats är bland annat att det finns tveksamhet kring teknologiernas lönsamhet och att personal tycker de tar upp alltför mycket tid. Det har dessutom rapporterats om svårigheter i post-implementationsfasen, till exempel ökad belastning för personalen och sämre kommunikation mellan personal och patienter. Med bakgrund i detta har denna studie undersökt vilka faktorer som påverkar användaracceptans av IT inom hälso- och sjukvård under post-implementationsfasen. Denna studie har med en kvalitativ ansats och med utgångspunkt i teorin UTAUT genomfört ett antal intervjuer med personal inom hälso- och sjukvård som nyligen varit med om en implementering av ny teknologi. Därefter har resultatet analyserats för att ta reda på de faktorer som mest påverkar acceptansen av teknologi inom hälso- och sjukvård. Resultatet visade att förväntad prestanda är den faktor som spelar störst roll för acceptansen av ny teknologi. Frivilligheten samt om användandet blivit norm på arbetsplatsen har stor påverkan på om en teknologi accepteras eller inte. Vid mer komplexa rutiner är det dessutom viktigt att känna till de processer som ligger till grund för en teknologi. Studien visar också att det finns ett behov att justera modellen UTAUT för att bättre förklara acceptans inom svensk hälso- och sjukvård. Graden av frivillighet bör vara mer än en modererande variabel, och en ny grundsten, som behandlar kunskap om de processer och rutiner som styr arbetet inom hälso- och sjukvård, bör betänkas.

Abstract [en]

Implementation of IT in healthcare is by many considered to have several benefits. On the one hand, it can improve the health care by, among other things, making it more efficient. On the other hand, it can give economic advantages, for example by health care providers being able to save money. At the same time, there are studies showing problems may arise at the implementation of new technologies in healthcare. Problems reported include hesitation regarding the profitability of the technologies, and staff deeming the technology taking up too much time. There have also been reported difficulties in the post-implementation phase, for example increased strain on personnel and worsened communication between staff and patients. In the light of this, this study has investigated which factors influence the user acceptance of IT in healthcare during the post-implementation phase. This study has, with a qualitative approach and based on the theory UTAUT, carried out a number of interviews with personnel working in healthcare who recently took part in an implementation of new technology. Then, the result was analyzed to find out which factors most influence the acceptance of technology in healthcare. The result shows that expected performance is the factor that matters most for acceptance of new technology. The voluntariness and if the usage has become norm in the workplace both have a large effect on whether the technology is accepted or not. When the work routine is of a more complex nature it is also important to have a good understanding of the processes which the technology is built around. The study also shows that there is a need to adjust the UTAUT model to better explain acceptance within the context of Swedish healthcare. The degree of voluntariness should be more than a moderating variable, and a new construct, regarding knowledge about the processes and routines that govern the work in healthcare, ought to be considered.
