Abstract [sv]

Bakgrund: Socioekonomiska skillnader ökar i Sverige och personer som lever i socioekonomisk utsatthet har en större risk att utveckla både fysisk och psykisk ohälsa. De som lever i socioekonomisk utsatthet har oftare en sämre tillgång till samhällets vård. Distriktssköterskans ansvarsområde är att arbeta hälsofrämjande för att förebygga ohälsa samt vara uppdaterad om hur olika samhälleliga förhållandens påverkan på hälsan. Detta för att kunna arbeta för en jämlik hälsa, utjämna skillnader och minska social och ekonomisk orättvisa. 

Syfte: Syfte med studien är att beskriva distriktssköterskors erfarenheter av att arbeta hälsofrämjande med patienter som lever i socioekonomisk utsatthet.

Metod: Studien genomfördes med en kvalitativ ansats och är baserad på semistrukturerade individuella intervjuer. Åtta distriktssköterskor intervjuades och intervjuerna analyserades med kvalitativ innehållsanalys med induktiv ansats.

Resultat: Fem teman framkom ur analysen: Distriktssköterskors erfarenhet av kvinnors sårbarhet, Frustration över låg kunskapsnivå och bristande kontinuitet, Tidskrävande patientgrupp, Ekonomiska begränsningar, och Distriktssköterskans ensamarbete. Distriktssköterska delade erfarenhet om att patienterna hade bristande kunskap om faktorer som påverkar hälsan. Patienternas bristande kunskap om hälsa och sjukdomsinsikt hade en betydande faktor till varför patienterna inte sökt vård tidigare samt resulterade i långa och återkommande besök. Distriktssköterskorna beskrev att hälsofrämjande arbetet var viktig men att bristande stöd i organisationen var orsaken till att det inte prioriterades. Att nå ut till kvinnor i det hälsofrämjande arbetet beskrevs som utmanade då de fångades upp i samband med andra besök

.Slutsats: Distriktssköterskans hälsofrämjande arbete med patienter som lever i socioekonomisk utsatthet är tidskrävande. Det behövs stöd och struktur på organisatorisk nivå samt bättre samverkan inom andra professioner då det upplevdes som mycket ensamarbete.

Abstract [en]

Background: Socio-economic differences are increasing in Sweden and people living in socio-economic vulnerability have a greater risk of developing both physical and mental illnesses. Those who live in socio-economic vulnerability are more likely to have more limited access to health care. The district nurse's area of responsibility is working towards promoting health in order to prevent illness and to be up to date on how different social conditions impact on health. This is to be able to work for equal health, balance out differences and reduce social and economic injustice.

Aim: The aim of the study is to describe district nurses' experiences of working to promote health with patients living in socio-economic vulnerability.

Method: The study was conducted with a qualitative approach and is based on semistructured individual interviews. Eight district nurses were interviewed, and the interviews were analyzed with qualitative content analysis with an inductive approach

Results: Five themes were analyzed in the data: District Nurses' experience of women's vulnerability, Frustration over low level of knowledge and lack of continuity, time-consuming patient group, financial constraints, and the District Nurse's lonelywork. The district nurse shared that patients had a lack of knowledge about factors that affected health. The patients' lack of knowledge about health and disease insight was a significant factor as to why patients did not seek care earlier and resulted in long and recurring visits. The district nurses described that health promotion work was important but that lack of support in the organization was the reason why it was not prioritized. Reaching out to women in health promotion work is described as challenging as they were caught up in connection with other visits.

Conclusion: The district nurse's health promotion work with patients living in socioeconomic vulnerability is time consuming. There is a need for support and structure at the organizational level as well as better collaboration within other professions as it was experienced as a lot of lone working.
